Overview
We are seeking a dedicated and skilled ANP or Paramedic Practitioner to join our clients Home visiting team on a long term locum contract. You will be working alongside a MDT covering visits mainly at various Care Homes and work closely with the Frailty/Care Home team. This work with be via a PCN so most of the visits / ward rounds will centre around Primary Care issues.
Responsibilities
- Visits and ward rounds at Care Homes
- Collaborate with other healthcare professionals to ensure comprehensive care for patients.
- Maintain accurate patient records and documentation in compliance with legal and organisational standards.
- Conduct thorough assessments of patients’ medical histories and current conditions.
- Participate in ongoing training and professional development to stay current with best practices in emergency medical services.
Qualifications
- Valid NMC / HCPC Pin
- Previous Experience in Primary Care desirable but not essential
- Independent Prescribing desirable but not essential
- Proven experience in community patient care, ideally including visiting shifts
- Excellent communication skills, both verbal and written, with the ability to work collaboratively within a team.
- A valid driving licence , as travel is required

Pay: £35.00-£50.00 per hour
