Your role and responsibilities
You will play a key role in shaping and improving how care services are delivered, working closely with colleagues, providers and partners to support better outcomes for people across Suffolk.

Work closely with hospital discharge teams, social workers, contracts and Home First teams to enable safe and timely discharges, reducing delays across the wider health and social care system.

Support the planning and implementation of systems that improve service flow and operational efficiency.

Monitor vacancies and capacity to improve access to residential and home care options, enabling informed choice and reducing delays.

Review outstanding care requests, using data and feedback to identify trends, inform decision-making and support continuous improvement.

Provide flexible support to meet service demand, including occasional weekend working.
You will need
-
NVQ Level 4 in Business Administration (or equivalent) or demonstrable relevant experience.

Strong literacy and numeracy skills.
-
An understanding of health and social care settings (desirable).
-
Experience of reviewing and improving processes and procedures.
-
Excellent attention to detail and accurate record-keeping skills.
-
Confidence using Microsoft Office and digital systems.
-
Strong communication, negotiation and influencing skills, with the ability to engage others clearly and empathetically.

The ability to remain resilient and adaptable in a fast-paced environment.

Good organisational skills, with the ability to prioritise and meet deadlines.

The team
The Brokerage Team plays a central role in sourcing care services for people with eligible needs. Acting as the main point of contact for social care placements across Suffolk, the team works collaboratively with Adult and Community Services, care providers and health partners to ensure timely access to care and support.
The team is focused on reducing delays, promoting independence and safeguarding vulnerable individuals, ensuring that support is arranged as quickly and effectively as possible. This work is delivered in line with key directorate models, including Supporting Lives, Connecting Communities and Signs of Safety.
