Job Summary
The Wellbeing Hub Community Connector plays a pivotal role in empowering individuals to live their best lives by fostering a supportive and resilient community environment. This role focuses on identifying, promoting, and connecting people to the local opportunities that support mental health and wellbeing, with a strong emphasis on addressing health inequalities and preventing escalation to statutory Services. You will build strategic relationships within the community, champion local initiatives, and facilitate co-design with those who have lived experience to create meaningful and accessible support.
Responsibilities
1. Community Engagement & Asset Development:
- Actively work within the community to identify and promote local opportunities that independently support mental health and wellbeing, with an understanding of the local population and a focus on addressing health inequalities.
- Connect and communicate with local communities to support the implementation and expansion of the local mental health and wellbeing offer (e.g., Recovery College, Mental Health Support Groups).
- Create and promote opportunities for social interaction, volunteering, and community activities to reduce isolation, create meaningful occupation, and build stronger communities.
- Lead in pioneering, establishing, and supporting the local Wellbeing Hub offer, including exploring, scoping, and building networks to extend provision.
- Encourage communities to recognise and utilise individual skills and community resources, fostering connections between individuals of all cultures and backgrounds for a more integrated and resilient community.
- Encourage signposting and access for diverse individuals and communities to a wide range of activities, including arts, crafts, physical activity, sports, cultural, and life-enhancing skill development.
2. Supporting Social Action & Community Empowerment:
- Build on existing levels of social action and community willingness to support new or established community groups and activities.
- Identify and establish linkages with key community leaders and individuals, building rapport to identify solutions for skills development, community activities, and new connections.
- Facilitate grassroots problem-solving and implement an asset-based approach to strengthening community resilience (e.g., community safety, older people's independence, youth outcomes, loneliness, early mental health support, food/fuel poverty).
- Develop community-led events and activities (online and offline) to engage and connect communities to identify and address their needs using available community assets.
3. Collaborative Working & Partnership Development:
- Collaborate with colleagues within the wider local mental health offer, local authority, NHS, voluntary sector partners, mental health and community services, faith groups, and business links.
- Work to present unified responses that solve problems early for individuals and communities, recognising the complexity of people’s lives and promoting independence.
- Support the involvement of people with lived experience of mental health in the design, delivery, and monitoring of support and activities.
- Act as a strategic partner to bridge the gap between communities and services, facilitating the translation of policy into meaningful action and fostering co-design.
- Lead on the promotion and strategic development of the community partnership approach, including facilitating forums, gathering intelligence on provision (gaps, inequalities, barriers), and coordinating joint bidding for funding.
- Connect communities and partners to projects, programmes, and activities, driving peer-to-peer support for community groups and leaders.
4. Data Management & Service Improvement:
- Identify and map community assets to update and maintain local directory/navigation information (e.g., HAY Lincolnshire) and support the ongoing development of the local mental health offer.
- Support regular audits by working with the HAY lead to ensure accurate and up-to-date information.
- Be responsible for updating individual ‘Meet the Connector’ information for the HAY lead.
- Develop procedures and systems for project delivery, capturing required data and information for annual and quarterly reports.
- Attend meetings and panels to support service development and contribute to service evaluations.
5. Health & Safety and Confidentiality:
- Demonstrate awareness and understanding of equal opportunities and the diverse behavioural, physical, social, and welfare needs of others.
- Ensure support reaches underserved, vulnerable, deprived, and isolated members of society, identifying ways to enhance access and reduce barriers.
- Maintain confidentiality of all information in accordance with policies, ensuring compliance with GDPR and patient confidentiality.
- Take reasonable care for the health, safety, and welfare of self and others, complying with health and safety policies and procedures.
Skills & Experience:
Essential:
- Proven experience in community engagement, development, or similar outreach roles.
- Strong understanding of mental health and wellbeing needs, preferably within a community context.
- Ability to build rapport and establish effective relationships with diverse individuals and groups.
- Excellent communication and interpersonal skills, capable of influencing and collaborating with various stakeholders.
- Experience in identifying and developing community assets and resources.
- A commitment to addressing health inequalities and promoting inclusion.
- Ability to work independently and as part of a wider team.
- Proficiency in data capture and reporting.
- Full UK Driving Licence and own transport
Desirable:
- Knowledge of asset-based community development (ABCD) principles.
- Experience in co-designing services with individuals with lived experience.
- Familiarity with local services and support networks in Grantham.
- Experience in facilitating community-led events or problem-solving initiatives.
- Experience in coaching or training others.
