Job Title: PCN Pharmacy Technician
Contract type: Permanent
Pay: Starting from £30,000 Per annum/pro rata if part time (dependant on experience)
Hours per week: Full time 37.5 hours per week (Part time can be considered)
Location: Remote and Practice based - the post-holder will be expected to be working at the surgery, as well as remotely, depending on needs of the business.
Job Overview
The post-holder will be working collaboratively with the PCN multidisciplinary team to support a safe and effective medicine management system. They will be assisting in improving prescribing process, supporting medication safety, and ensuring accuracy in medicines management systems within practice(s). They will liaise with patients and clinicians to resolve any medication queries and improve continuity of care. In addition, they will support clinical pharmacists and GPs in delivering national and local prescribing priorities such as compliance with prescribing policies and managing NPSA alerts. They will also be contributing to quality improvement initiatives, patient safety programme and service development within the practice(s)/PCN.
They will be working across two surgeries – Orchard Surgery and Roysia Surgery.
Primary/Core Responsibilities
The following are the core responsibilities of the Clinical Pharmacy Technician. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
a) Medicines Reconciliation:
- To reconcile medicines following patients discharge from hospital or clinic appointments
- Review medications for newly registered patients
- Identify discrepancies and resolve medication queries with appropriate clinicians
- Update and maintain accurate medication record
- Contact patients ensuring they are aware of the changes made to their medicines
- Support patients with understanding their repeat prescriptions and ordering process
- Provide medicines counselling, ensuring patients have good understanding with administration instructions and awareness of side effects, especially medicines that are time-sensitive and/or has specific administration requirement
- Address public health and social care need including lifestyle advice. Where competent signpost patients to appropriate clinicians/organisations
b) Medication Safety and Quality Improvement:
- To manage high risk medications monitoring and recall of patients using appropriate platform (e.g. Eclipse/Ardens)
- Assist in monitoring Long Term Conditions through medicines-related interventions
- Review all local and national medication safety alerts ensuring the required actions are taken
- Review and implement safe prescribing systems for vulnerable patients, improving compliance
- Participate in clinical audits and quality improvement projects
- Support achievement of QoF, IIF and local prescribing targets as required
General Responsibilities
In addition to the primary responsibilities, the Clinical Pharmacy Technician are expected to:
- Maintain registration with General Pharmaceutical Council
- Keeping up-to-date with professional responsibilities and CPD requirements
- Work within their professional competence and seek supervision when required
- Maintain patient confidentiality and adhere to information governance requirement
- Participate in supervision, appraisals and team meetings

Qualifications:
- GPhC Registered Pharmacy Technician (E)
- BTEC/NVQ Level 3 in Pharmaceutical Science or equivalent (E)
- Completed CPPE Primary Care Pathway (or equivalent) (D)
- Completed Level 4 Pharmacy Technician training (D)
Experience:
- Minimum of two years working as a Pharmacy Technician (E)
- Experience in General Practice/PCN (D)
- Broad knowledge of General Practice and Primary Care (D)
- Broad knowledge of secondary care (D)
