Hospital Navigator

Substances Support Scotland (SSS)
Victoria Hospital, Kirkcaldy (A&E Department)
£28,061 per annum
36 hours per week
Fixed-Term Contract – 12 Months (subject to funding)

Change Starts With One Conversation

Substances Support Scotland (SSS) is looking for a compassionate, motivated and dedicated Hospital Navigator to join our ASSIST Service based within Victoria Hospital, Kirkcaldy.

This is an exciting opportunity to support individuals experiencing problematic alcohol and substance use who are often facing additional challenges including homelessness, housing instability, poor mental health, social isolation and financial hardship. The role is focused on helping people move from crisis towards stability, wellbeing and lasting change through practical support, advocacy and community navigation.

About the Role

As a Hospital Navigator, you will work within the A&E setting and across community services, providing intensive short-term support to individuals affected by alcohol and substance use.

You will:

Build positive and trusting relationships with individuals experiencing alcohol and substance-related harm.
Undertake holistic assessments and support planning.
Deliver person-centred, trauma-informed and strengths-based interventions.
Support individuals experiencing homelessness or housing instability.
Assist people to access healthcare, housing, welfare benefits and community resources.
Advocate on behalf of individuals to ensure they receive the support they need.
Work collaboratively with statutory and third-sector partners.
Facilitate onward referrals into Dedicated Addiction Worker support where appropriate.
Maintain accurate records and contribute to a high-quality support service.

What We’re Looking For

Essential

SVQ Level 3 in Health & Social Care (or equivalent recognised qualification).
Full UK Driving Licence.
Experience supporting individuals affected by alcohol and/or substance use.
Experience working with vulnerable individuals experiencing multiple disadvantage.
Experience in assessment, support planning and case management.
Experience working within multi-agency partnerships.
Understanding of trauma-informed and person-centred practice.
Strong communication, advocacy and relationship-building skills.
PVG Membership or willingness to join the PVG Scheme.

Desirable

Experience delivering outreach or community-based services.
Experience working within recovery communities.
Experience supervising volunteers.
ABI, DBI, ASIST, SMHFA, Naloxone or Trauma-Informed Practice Training.
Motivational Interviewing experience.
