Job overview

We are seeking a motivated and compassionate Band 5 Registered Nurse to join our dynamic Discharge Lounge team.

The Band 5 Nurse in the Discharge Lounge plays a key role in supporting safe, timely, and effective patient discharge from hospital. The post holder is responsible for coordinating and delivering nursing care to patients awaiting discharge while ensuring that all necessary arrangements are in place before they leave the hospital.

You will have excellent communication skills, clinical competence, and a passion for supporting and developing colleagues. Opportunities for professional development, including mentoring, shadowing, and skills training, are available.

We are recruiting for two posts - 1 x full time; 1 x part time

Main duties of the job

To provide safe, high-quality, patient-centred nursing care within the Discharge Lounge, ensuring patients are appropriately assessed, monitored, and prepared for discharge. The post holder will work closely with the multidisciplinary team to facilitate timely and safe patient discharges, provide discharge information and support to patients and carers, administer medications as required, maintain accurate documentation, and contribute to the effective flow of patients through the service while promoting a positive patient experience.
