The Fairfields Practice is an established inner-city practice based at The Mary Potter Centre. We provide medical services for 7500 registered patients.
We are seeking a care coordinator to join our multi-disciplinary team.
o Utilise population health intelligence - GP clinical system, Ardens (templates, searches, and manager), and eHealthsope to proactively identify and work with a cohort of patients to deliver personalised care holistically. To co-ordinate the bring together all of a persons identified care and support needs and explore options to meet these within a single personalised care and support plan in line with best practice.
o Help people to manage their needs through answering queries, making, and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care.
o Maintain an up-to-date knowledge of the clinical and reporting requirements of all clinical contracts entered into by the practice (QoF, IFF, Enhanced Services etc).
o Run Ardens searches, construct and run searches if needed to coordinate recall activities to ensure the identification of eligible patients (in accordance with contractual or national guidelines) and ensure they are recalled into the practice.
o Use text messaging services to send campaign messages to patients.
o Maintain and adapt the chronic disease recall programme to ensure patients are recalled in a timely, efficient way, ensuring an even spread of workload as much as possible.
o Liaise with colleagues with QOF IIF responsibilities to track progress and identify trends or areas of concern which might cause performance targets to be missed and report to the Practice Manager / Partners.
o Liaise with clinical and administrative colleagues as appropriate to ensure templates, searches, protocols, and any other clinical IT appropriate tools are implemented, maintained, fit for purpose, and updated in a timely manner.
o Periodically undertake checks to ensure the quality of data entry, including consultation data and correct use of clinical codes is maintained and report any deficiencies to Partners or management as necessary.
o Assist with practice research
o Carry out both planned and ad hoc audits of clinical activity as requested by the Partners/Practice Manager or as dictated by contractual obligations to establish standards of care and target areas for improvement. This may require interrogation of data provided by third parties and reviewing entries in patient records as appropriate.
o Carry out validation checks on data supplied by other healthcare organisations as requested and investigate and report any inappropriately recorded activity.
o To become the practice expert in all aspects of the clinical system and be able to train and support other users, both clinical and non-clinical, in the best use of the system
o Coordinate and lead on patient online services
o As requested by the practice manager or Partners, prepare statistical or data reports (excel) support the coordination and delivery of MDTs within the PCN.
Person SpecificationQualificationsEssential
- GCSE grade A to C in English and Maths or equivalent
- A levels or equivalent

- Degree in STEM subject or equivalent

- Relevant experience in a similar role, involving data and the need for accuracy.
- Experience of multi-tasking in a complex and demanding environment.
