The Keeping Families Connected Service works intensively with children and their families, and are closley aligned to the Safe and Supported Social work teams.
Referrals to the service are based on 3 core objectives-
Prevent children becoming cared for by the local authorty
Stabilise living arrangments for children at risk of disruption and breakdown
Reunify children home, when it is safe to do so.
The Keeping Families Connected service is a multi-disciplinary team that adopts a trauma-informed approach, using clinical formulation to guide and shape the work undertaken. The service works intensively for a period of up to 16 weeks and offers 7 day a week provision to meet the needs of the child and family.
The service was assessed by OFSTED in 2025 - OFSTED said "Exceptionally strong practice was seen by inspectors in the Keeping Families Connected service. This servce works effectively and successfully with children and families in helping to maintain or reunite children with thier birth families safely and appropriately This provision of support is proportionate to the needs of families is helping to safely prevent children entering care.
The successful candidate
We are looking for a skilled and organised practioner who can confidently engage children, young people and families, particularly during times of crisis, and deliver high quality,outcome focused support. You'll bring strong assessment and planning skills, alongside a solid understandiing of intervention approaches and multi-agency working, to help drive meaningful change.
