To strength the leadership teams within our home this role is required to cover the 7 day week, including weekends. The postholder will work on average 5 days per week on a rota basis, working on average 2 weekend days every three weeks.

Job Purpose
The Care Coordinator plays a key role in ensuring the effective planning, coordination, and delivery of high-quality, person-centred care within the care home. The role is responsible for developing, reviewing, and maintaining care plans, coordinating communication between residents, families, healthcare professionals, and care teams, and ensuring that residents' changing needs are identified and responded to appropriately.

The role requires a strong visible presence within the home, regularly spending time on the floors to monitor care delivery, support staff, engage with residents and relatives, and ensure care plans accurately reflect residents' needs. The Care Coordinator is also responsible for producing and maintaining care staff rotas to ensure safe and effective staffing levels across the service.
Core Responsibilities
Care Planning & Coordination
- Develop, implement, review, and maintain person-centred care plans and risk assessments that accurately reflect residents' needs, preferences, choices, and outcomes.
- Conduct regular care plan reviews and update documentation following changes in residents' physical, emotional, social, or clinical needs.
- Work closely with residents, families, nurses, care staff, and external professionals to ensure coordinated and effective care delivery.
- Ensure all care records are accurate, comprehensive, and compliant with regulatory and organisational requirements.
- Support the admission, transfer, and discharge processes, ensuring continuity of care and effective communication throughout.
Rota Management & Workforce Coordination
- Prepare, manage, and maintain weekly and monthly care team rotas to ensure safe staffing levels and appropriate skill mix across the home.
- Monitor staffing requirements and proactively address rota gaps, annual leave requests, sickness absence, and temporary staffing requirements.
- Work collaboratively with management to ensure staffing resources are utilised effectively and within budgetary requirements.
- Maintain accurate staffing records and ensure rotas comply with organisational policies and employment legislation.
- Support effective shift planning and deployment of staff to meet residents' needs and ensure continuity of care.
Visible Leadership & Floor Presence
- Maintain a regular and visible presence on the floors, observing care delivery and supporting staff in providing high-quality, person-centred care.
- Build positive relationships with residents, relatives, and team members, ensuring concerns, feedback, and suggestions are listened to and acted upon.
- Monitor standards of care and documentation through daily engagement with care teams and residents.
- Support and coach care staff in understanding and implementing care plans effectively.
- Act as a positive role model, promoting professionalism, accountability, and compassionate care throughout the service.
Quality Assurance, Improvement & Governance
- Ensure care documentation remains accurate, up to date, and inspection-ready at all times.
- Support the implementation of action plans arising from audits, inspections, incidents, and quality reviews.
- Assist in preparing for regulatory inspections and maintaining compliance with CQC standards and organisational policies.
Communication & Partnership Working
- Act as a key liaison between residents, families, care staff, nurses, healthcare professionals, and external agencies.
- Communicate effectively and professionally, ensuring information is shared accurately and appropriately.
- Support residents and relatives to understand care plans and participate in decisions regarding care and support.
- Ensure confidentiality is maintained in line with GDPR and organisational policies.
Safe Care & Safeguarding
- Demonstrate a clear understanding of safeguarding adults, Mental Capacity Act, Deprivation of Liberty Safeguards, and related legislation.
- Support staff to identify and report safeguarding concerns appropriately.
- Promote safe systems of work and contribute to effective risk management throughout the home.
- Escalate concerns relating to resident safety, wellbeing, staffing, or care quality promptly and appropriately.
- Support investigations, incident reviews, and learning outcomes where required.
Staff Support & Development
- Provide guidance and support to care staff regarding care planning, documentation, and best practice.
- Assist with the induction and ongoing development of new team members.

Person Specification (Summary)
Essential:
- Experience working within a care home, healthcare, social care, or care coordination environment.
- Strong understanding of person-centred care planning, risk assessments, and regulatory requirements.
- Experience creating and managing staff rotas and coordinating workforce deployment.
- Excellent organisational, communication, and time management skills.
- Ability to build positive relationships with residents, families, staff, and healthcare professionals.
- Strong IT skills, including electronic care planning systems and Microsoft Office applications.
- Understanding of safeguarding, confidentiality, and relevant health and social care legislation.
Desirable:
- NVQ Level 3 or above in Health & Social Care or equivalent qualification.
- Previous experience as a Care Coordinator, Senior Care Assistant, Team Leader, or similar role within a care home setting.
- Knowledge of CQC regulations and inspection requirements.
- Experience supporting quality improvement initiatives and audits.
- Experience working within dementia care and older people's services.
- Understanding of workforce planning and staffing management within a care home environment.
