JOB SUMMARY: Elvington Medical Practice is looking for a Practice Nurse to join the team who has experience of managing and treating patients with a variety of long-term health conditions in addition to general practice nursing skills. You will be committed to delivering outstanding care to our patients and will work closely with doctors and multi-disciplinary team members. The ideal candidate will be enthusiastic, professional, flexible and importantly a team player with a ‘can do’ attitude.
ABOUT US: We are a friendly, busy, semi-rural dispensing practice based outside York offering 'Good' CQC rated service. We are a teaching practice and regularly have student doctors with the focus on delivering high quality care to our 7200 patients.
Our clinical team comprises five GP partners, three salaried GPs, three practice nurses, a health care practitioner and a health care assistant. We also have a pharmacist and pharmacy technician with supportive dispensing and administrative teams. Through our PCN we have a mental health practitioner and a social prescriber. We use EMIS Web and are high achievers in all areas of QOF.
Key Responsibilities
Clinical delivery
- Implement and evaluate individual treatment plans for patients with long-term conditions
- Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self-care
- Deliver opportunistic health promotion
- Support and advise women requesting information relating to family planning needs
- Perform cervical screening as per national guidelines
- Participate in vaccination and immunisation programmes for children and adults. Always maintain the cold chain
- Assess and care for patients presenting with acute or chronic wounds
- Assist doctors with minor surgery and LARC procedures as required
- Ability to perform ECGs
- Advise, support and where appropriate administer vaccinations for patients travelling abroad
- Monitor work areas and practice ensuring that they are safe adhering to current health, safety and security guidelines
- Apply infection control measures within the practice and carry out audits and checks as per local and national guidelines.
Communication
- Treat people with kindness, respect and compassion
- Utilise effective communication styles to ensure patients are fully informed and consent to treatment
- Use communication skills to support patients to adhere to prescribed treatment plans
- Anticipate barriers to communication and take appropriate action to improve communication
- Act as an advocate for patients and colleagues
- Attend team meetings
- Document accurately patient consultations as soon as possible after the event
- Manage and assess risk within given areas of responsibility, ensuring adequate measures are in place to protect patients and staff
- Ensure clear understanding and utilisation of referral mechanisms within the practice
- Use technology as an aid to management in planning, implementation, monitoring, presenting, and communicating information
- Ensure confidentiality is always maintained as per Caldicott requirements.
