Sheringham Medical Practice are looking for a motivated and enthusiastic, Patient Care Coordinator to work alongside other members of our friendly and dedicated, multi-disciplinary team.
The practice has an excellent multi-disciplinary team who provide and maintain a high standard of care for our patients and are supported and led by an enthusiastic partnership and management team.
The successful candidate will work alongside a friendly reception team who are very supportive, caring and who pride themselves with providing excellent services to our patient population.
Summary of Responsibilities
The role will be based in Sheringham Medical Practice, which is a 6-doctor dispensing practice providing care to around 9,500 patients in and around Sheringham in North Norfolk.
The post holder will be responsible for providing a high quality, professional administrative service to patients, doctors, nurses, staff, colleagues, health service professionals and others.
The Patient Care Coordinator will act within their professional boundaries, working alongside the general practice team. The main aim of the role is to work for the benefit of patients, providing and maintaining a high standard of care for patient’s health needs.
All staff must follow the professional codes of practice. This means that they must make every effort to protect confidentiality. It also means that no identifiable information about a patient is passed to anyone or any agency without the express permission of that patient. A breach of confidentiality will result in dismissal.
As part of the role, you will be required to complete three modules set out by the Personalised Care Institute.
Main duties of the job
- To oversee and coordinate patient requests via the digital system.
- Act as a point of contact between the practice, patients and carers and other agencies
- Liaise with HCAs, Nurses GPs and practice teams to identify patients who have long term health needs and support
- Complete and action tasks for the administration team ensuring they are actioned appropriately
- Support the practice with administrative tasks associated with patient care
- Undertake telephone contact to manage patients on the practices case load following any unplanned hospital admissions where appropriate.
- Raise awareness of health promotion, screening and NHS Health Checks and LD Health Checks in practices
- Co-ordinate practice clinics, and contact identified patients with appointments
- Identify, where required and as appropriate, patient who need to be referred to other health professionals within the practice and externally using the patient protocol
- To help people to manage their needs through answering queries, making, and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care.
- Work closely with and refer to practice social prescribing link workers where a patient is identified as potentially benefitting from this service.
- Process, record and oversee incoming personal data requests and action accordingly.·
- Administration and communication of completed Do Not Attempt Resuscitation (DNAR) proforma.
- Daily review of notifications and tasks, using judgement to take appropriate action.
- Registration of new and temporary patients
- Identifying registering carers signposting and supporting with resources available
- Scanning of documents and addition to patient records as required e.g. blood donor forms, organ donor forms.
- Process Third party consent forms
- Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing.
- Provide the interface between primary care and other services, helping patients with the transition between services.
- Ensures awareness of statutory and local clinical protection procedures, including systems of referral.
- Attend and participate in any practice meetings when required
- Prepare premises punctually in the morning.
- Ensure you are properly logged on to the computer system under the correct usernames and passwords.
- Answer the telephone courteously and efficiently, taking and recording messages accurately ensuring calls are documented in the SystmOne computer system and signposted accordingly according to the patient services protocol.
- To receive patients and other members of the public in person and by telephone politely.
- All staff are expected to comply with local policy and protocol including the Staff Core Values.
- Ensure patients are correctly checked in for their appointment and educate patients in the use of the self-check in system.
- Ensure all entries to the appointment screen of the SystmOne are accurate.
- Ensure all tasks are recorded on the SystmOne computer system.
- Confirm and or amend patient appointments for practice-based appointments or clinics.
- Ensure all mail is opened and distributed to the data entry team to be processed.
- Ensure an adequate supply of stationary and other routine documents are available in the reception.
- Undertake administrative tasks as requested.
- Ensure the reception area is clean, tidy, and presentable at all times.
- Responsible for end of day check, lock, and secure premises at night as and when directed, checking that the telephone system has transferred correctly to the Out of Hours service, providing emergency dispensing.
- Report to the appropriate Manager any matter affecting patient confidentiality, safety, or security.
- Providing patients with test results reported on by a clinician

Expected hours: No more than 24 hours per week. The successful candidate will need to be flexible and willing to work hours around the practice opening hours, which are:
Monday :08:00-20:00
Tuesday: 08:00-18:30
