Job Summary
East Leicester Medical practiceis excited to be recruiting to this key role into our friendly team. We are keen to identify a Nurse Associate to join our dedicated team of Practice Nurses and Health Care Assistants.

We serve 13k patients employing around 45 staff, including 2 Practice Nurses, 3 Health Care Assistances and 1 Phlebotomist.

We are looking for a nurse Associate to join our team who is able to work autonomously within their scope of practice and collaboratively with the multidisciplinary team to support the nursing team.

The Nursing Associate will play a role in supporting our practice population to improve and maintain their mental, physical, behavioural health and wellbeing.

The Nurse associate will actively support in the prevention of and protection against disease and ill health, and engage in public health, community development, to aid in the reduction of health inequalities within our practice population.

The Nurse Associate will provide compassionate, safe, and effective care and support to patients in a general practice setting.

Responsibilities

Ø You will also be required to take vital signs and observations and identify signs of improvement, report any deteriorations or concerns, e.g. pulse, blood pressure, respirations, oxygen saturations, peak flow, blood glucose levels, ECG, assessing nutrition and hydration and undertake pathology tests, for example urine culture, swabs and H pylori breath testing.

Ø Monitor the condition and health needs of patients within their care on a continual basis in partnership with people, families, and carers.

Ø Contribute to ongoing assessment and can recognise when it is necessary to refer to others for reassessment

Ø Enable delivery of person-centred safe and effective care

Ø Recognise and apply knowledge of commonly encountered mental, physical, behavioural, and cognitive health conditions when delivering care.

Ø Clinical IT system user skills

Ø Demonstrate the knowledge, communication and relationship management skills required to provide people, families and carers with accurate information that meets their needs before, during and after a range of interventions

Ø work in partnership with people, to encourage shared decision making, in order to support individuals, their families and carers to manage their own care when appropriate

Ø demonstrate the knowledge, skills and ability to perform a range of nursing procedures and manage devices, to meet people’s need for safe, effective and person-centred care

Ø demonstrate and apply an understanding of how and when to escalate to the appropriate professional for expert help and advice.

Ø demonstrate and apply an understanding of how patient’s needs for safety, dignity, privacy, comfort, and sleep can be met

Ø Assist with baby immunisations and vaccination clinics

Ø Perform ECG’S and cervical smears.

Experience

Ø Experience of working in teams under appropriate supervision as part of a multi-disciplinary team.

Ø Insight into how to evaluate own strengths and development needs, seeking advice where appropriate.

Ø Experience of providing and receiving complex, sensitive information.

Ø Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development.

Ø Understands and acts in line with NMC professional standards for practice contained within The Code.

Ø Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice.

Ø Understand requirements for NMC professional revalidation.

Ø Understanding of the importance of following procedures and treatment plans.

Ø Ability to take part in reflective practice and clinical supervision activities.

Ø Ability to organise and prioritise own delegated workload

Ø Ability to deal with non-routine and unpredictable nature of workload and individual patient contact.

Ø Ability to communicate effectively (written, verbal and non-verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team.

Ø Ability to develop effective and appropriate relationships with people, their families, carers and colleagues.

Ø Knowledge of community based primary care issues.
