We are seeking a dedicated Practice Nurse to join our healthcare team. The successful candidate will play a vital role in delivering high-quality patient care within a busy clinical environment. The Practice Nurse will be responsible for providing comprehensive nursing services, supporting doctors and other healthcare professionals, and ensuring the smooth operation of daily clinical activities.
Responsibilities
- Provide optimum level of patient centred care for health promotion, screening, chronic disease management and general nursing procedures
- Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
- Assess, diagnose, plan, implement and evaluate treatment programmes that promote health and wellbeing
- Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition within own competence
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
- Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
- Promote and deliver evidence-based care for patients presenting with aural conditions.
- Meet the needs of patients presenting for opportunistic and planned wound care.
- Manage and administer immunisations under Patient Group Directions and Patient Specific Directions and provide relevant information to patients.
- Work within own level of competence in accordance with NMC regulation referring to GP when appropriate.
- Apply national and local policy and clinical guidelines/protocols to care delivery
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and work with patients in order to support adherence to prescribed treatments
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation.
- Recognise, assess and work with medical colleagues to support patients presenting with mental health needs
- Ensure that patient data is accurately recorded according to practice policy and NMC regulation.
- Adherence to CQC regulations and the Practice Infection Control Policy reporting any areas of concern to the Infection Control lead nurse

South Street is a large innovative training practice based across 3 sites in Bishops Stortford (South Street Surgery, Bishop's Park Health Centre and Herts and Essex Hospital) and providing care to approximately 24,000 patients.
We work closely with the other local practices in Stort Valley and Villages PCN to develop our vision for the future of Primary Care Services in this area.
