Full job description

As a key member of the multiple sclerosis team within Frimley Health Foundation Trust, you will work directly with the neurology Consultants, MS Consultant Nurse, MS Nurses and MS co-ordinator.

You will also be part of the wider neurology team within the integrated care system covering Frimley Park, Heatherwood Hospital and Wexham Park.

This role is based at Frimley Park however you will be expected to travel to different sites as required and carry out home visits

Safe delivery of MS therapies and management of multiple sclerosis patients across the whole disease trajectory from diagnosis to end of life.

Facilitating treatment pathways of emerging/established NICE approved MS therapeutics and also the care and support of all FHFT MS patients to include any task that benefits the neurology MS clinical service.

Providing home visits to patients who are unable to attend clinic appointments.

You will be involved in the wider MDT discussions/meeting and actively participate and on occasions lead these regarding the care of people with MS.

To participate in the provision of information/services to patients and their careers, regarding their disease, treatments and services available.

To promote independence and self-management of MS.

To support members of the multi-disciplinary team in the care of patients with MS.

Liaise with all relevant professionals and non-professionals in the planning of OPD clinics.

To attend and participate in MDT and other relevant meetings when required.

To participate in the provision of information/services to patients and their careers, regarding their disease, treatments and services available.

To administer disease modifying therapies safely, providing information about side effects to treatments, ensuring patient information is up to date and relevant.

To advise on other treatments as required such as symptom management therapies and provide advice and information on the management of these to patients, carers, district nurses, G.Ps and medical/nursing staff within the Trust.

To provide knowledge and advice in the management of physiological and psychological symptoms.

To interpret blood test results and act accordingly with regards to treatment.

To interpret clinical signs and symptoms and to monitor effect of treatment.

To participate and actively engage in the MDT process across the wider organisation.

To assess patients in their home environment and recognise the need for onward referrals to physio, OT, adult social care, safeguarding etc.

To co-ordinate investigations, treatments and arrangements for patients receiving therapies in accordance with established protocols and procedures.

To assist in the day to day running of the MS nursing service.

Seek support from colleagues when challenging situations arise.
