This 9-month contract role with a local authority is designed to support young people, particularly care leavers, in their transition to independent adulthood. The position involves providing holistic, person-centred advice, guidance, and intervention. The successful candidate will manage a caseload, deliver tailored support programmes, and collaborate with internal teams and external agencies to ensure young people access appropriate services, develop life skills, and achieve positive outcomes.

Act as a full member of the Service team and contribute to its development within the Directorate.

Manage an individual caseload and deliver direct support to young people on a one-to-one and group basis.

Participate in duty and referral work, responding to emerging needs appropriately.

Develop and implement tailored support programmes to assist young people transitioning from care to independent living.

Maintain regular contact with service users to support their development, independence, and self-esteem.

Conduct assessments and develop personalised support plans in conjunction with young people, social workers, carers, and partner agencies.

Ensure all young people receive their entitled services and support from relevant agencies.

Support access to education, employment, accommodation, and benefits.

Work collaboratively to support young people with continuing care needs and ensure a smooth transition into adulthood.

Experience working with vulnerable young people, particularly care leavers or those transitioning to independence.

Experience managing a caseload and delivering structured support interventions.

Proven experience in multi-agency partnership working.

Relevant qualification in Social Work, Youth Work, Social Care, or a related field (e.g., NVQ Level 3/4 or equivalent).

Evidence of continuous professional development.

Enhanced DBS clearance (required).

Working hours: 36.5 hours per week.

Location: One Avenue Road, Aston, Birmingham, West Midlands, B6 4DU, United Kingdom.

Work pattern: 2 days in the office, 3 days remote.

Bi-weekly schedule.
