Case Worker Temp Contract Fulltime
£13:50hr plus holiday pay
Cardiff

BRC are currently recruiting for a Case Worker to join a dedicated and supportive team delivering community based dementia support across Cardiff. This is a fixed term opportunity offering a rewarding and varied role where no two days are the same.
This is a fantastic opportunity to support individuals living with dementia to remain active, achieve personal goals, maintain independence, and improve their overall wellbeing.
Key responsibilities include:

Providing one to one support to individuals living with dementia
Completing assessments and developing person centred support plans
Supporting individuals to achieve meaningful goals and activities
Facilitating group activities within community and care settings
Providing practical and emotional support to families and carers
Working alongside volunteers, health professionals and partner agencies
Maintaining accurate records and case management systems

Experience:

Experience within health and social care, support work or community services
Experience supporting individuals with dementia is desirable
Ability to work independently and manage a varied caseload
Strong communication and relationship building skills
Full UK driving licence and access to own vehicle
Good IT and administration skills

This role would suit someone who is compassionate, proactive and passionate about supporting people to live well with dementia.
