Job summary
These exciting opportunities will enable you to gain more experience of supporting people to develop, retain or regain independent living skills that have been disrupted through the experience of living with mental illness.
Early Intervention in Psychosis Team You will support the service user with their mental and physical health using NICE recommended interventions for psychosis and to support service users in the transition from Inpatient to Community setting. Mental Health work experience is required for this role.
Assertive Outreach Team to work collaboratively with Care Coordinators to co-produce good quality assessments and interventions to service users who are experiencing severe mental illness and find it difficult to engage with mainstream services. This is 22.5 hours a week.
Homeless Healthcare-You'll build trusting relationships, offer day to day support, and help individuals access healthcare, housing, and recovery pathways supporting people facing homelessness, mental ill health, and substance use. Mon-Fri 8am-5pm. This is 30 hours a week.
Crisis resolution and home treatment team supports individuals who are experiencing acute mental health crisis and who require intensive input in order to remain in the community, averting hospital admission where possible. Whilst also supporting individuals along their journey through the Acute Care Pathway from the hospital environment to home. Service hours are Mon-Fri 9-5 or 2-10 with alternate weekends.
