Overview
An exciting opportunity has arisen for an experienced Advanced Nurse Practitioner (ANP) to join Fenland Primary Care Network (PCN) delivering a dedicated home visiting service for housebound patients and care home residents.
The role focuses on providing autonomous, high-quality care including holistic enhanced assessments, management of acute and long-term conditions, medication reviews, prescribing, and palliative care.
Fenland Primary Care Network (PCN) is a collaborative network of four GP practices, comprising three practices in March and one in Ramsey.
The PCN works together to deliver high-quality, patient-centred care to a diverse population, including a significant number of housebound patients, care home residents, and individuals with frailty and complex long-term conditions.
The ANP will deliver a dedicated home visiting service across the PCN, working autonomously within advanced clinical practice scope.
The role involves managing a caseload of housebound and care home patients, providing both planned and urgent care, and delivering holistic enhanced reviews, medication optimisation, and end-of-life care.
Responsibilities
Home Visiting
- Provide planned, routine, and urgent home visits
- Assess, diagnose, and manage acute and chronic conditions
- Respond to same-day clinical need
- Deliver ongoing monitoring and continuity of care
Holistic Enhanced Reviews
- Undertake comprehensive, patient-centred assessments including:
- Physical health
- Mental health and cognition
- Functional status
- Social and environmental factors
- Develop personalised care plans
- Identify and manage clinical risk and deterioration
Frailty & Complex Care
- Manage patients with frailty and multiple long-term conditions
- Provide proactive care for high-risk patients
- Optimise long-term condition management
Medication Reviews & Prescribing
- Undertake structured medication reviews, particularly in:
- Polypharmacy
- Housebound patients
- Care home residents
- Prescribe independently (V300) within scope
- Work with PCN pharmacy team to optimise medicines use
Care Home Support
- Provide cover for regular clinical input to care homes
- Undertake resident reviews and acute care assessment
- Participate in MDT and care home ward rounds (cover)
Palliative & End-of-Life Care
- Deliver high-quality end-of-life care
- Support symptom control and patient comfort
- Contribute to advance care planning and DNACPR decisions
- Liaise with specialist and community teams
Multidisciplinary Working
- Work collaboratively with PCN MDT:
- GPs, nurses, pharmacists, social prescribers
- Participate in MDT meetings and care planning
Qualifications
Essential Qualifications
- Registered Nurse (NMC)
- MSc Advanced Clinical Practice (or equivalent)
- Independent Prescriber (V300)

- Primary care, community, or home visiting experience
- Management of frailty and complex long-term conditions
- Medication reviews and prescribing
- Palliative and end-of-life care
