Join Our Team as a Pharmacy PCN Care Coordinator

General Practice Alliance (GPA) are seeking a proactive Pharmacy Care Coordinator to join our friendly team, working within Grand Union PCN, and helping to make a real impact on patient care.

This is a full‑time role, though part‑time hours will be considered.

Please note: We are unable to offer visa sponsorship for this role.

What you'll do:

In this role, you’ll support patients and our pharmacy team by helping people understand and manage their medicines, preparing them for clinical reviews, and ensuring they can access the right support at the right time. You’ll play an important part in coordinating care and helping maintain safe, effective medicines management across the PCN.
