We are looking for a highly organised, proactive and professional Service Delivery Scheduler to join our office team and play a key role in the day-to-day coordination of our care service.
This is an important role within the organisation. The Service Delivery Scheduler is responsible for ensuring that care visits are planned, covered, monitored and communicated effectively, helping to ensure clients receive the right care, at the right time, from the right staff.
As part of this position, the successful candidate will also participate in the Unisus Care Out-of-Hours and On-Call Service, supporting the safe delivery of care outside normal office hours on a rota basis.
About the Role
As a Service Delivery Scheduler, you will be responsible for preparing and maintaining staff rotas, allocating care visits, monitoring care delivery, responding to changes and supporting the safe running of the service.
You will work closely with the Registered Manager, Quality and Compliance Manager, Service Delivery Coordinator, Lead Community Care Practitioners and Community Care Practitioners to ensure care is delivered safely, consistently and in line with client needs.
This role requires someone who can remain calm under pressure, manage competing priorities, communicate clearly and make safe, practical decisions in a busy care environment.
The successful candidate will also be expected to participate in the organisation's Out-of-Hours/On-Call rota, providing operational support for urgent care issues, staffing emergencies and client concerns outside standard office hours.
Key Responsibilities
Your main duties will include:
- Preparing and maintaining care staff rotas to ensure all client visits are covered safely and effectively.
- Allocating Community Care Practitioners to visits based on client needs, staff skills, continuity, location, availability and service risk.
- Supporting consistent care runs to promote continuity for clients and staff.
- Monitoring the rota throughout the day and responding to sickness, emergencies, cancellations, late calls, changes in care needs and urgent cover requirements.
- Ensuring visit times are realistic and consider travel time, medication timing, client preferences and assessed care needs.
- Identifying rota gaps and taking prompt action to resolve them.
- Communicating professionally with clients, relatives, staff and external professionals regarding visit times, changes, delays or concerns.
- Monitoring electronic care systems to ensure visits are completed as scheduled.
- Escalating missed visits, late visits, medication concerns, safeguarding issues, staff conduct concerns or changes in client presentation.
- Maintaining accurate records of rota changes, communication, concerns, actions taken and follow-up required.
- Supporting a culture of safe, person-centred and responsive care delivery.
- Working in line with Unisus Care policies, safeguarding procedures, confidentiality requirements and CQC expectations.
- Participating in the Out-of-Hours/On-Call rota, responding to emergency staffing issues, client incidents, medication concerns, safeguarding concerns and operational emergencies outside normal office hours.
- Providing telephone support and coordination to Community Care Practitioners during on-call periods, ensuring continuity and safety of service delivery.
- Making safe operational decisions during out-of-hours periods and escalating concerns appropriately to senior management where required.
- Maintaining accurate records of all out-of-hours activity, actions taken and communications in accordance with company procedures.

Essential Requirements
The successful candidate must have:
- Strong administrative or coordination experience.
- At least THREE years' experience as a Care Coordinator or in a domiciliary care scheduling role.
- Access to a car and a valid UK driving licence.
- Excellent communication and organisational skills.
- Good IT skills.
- The ability to work in a fast-paced environment.
- A professional and respectful approach.
- An understanding of confidentiality and safeguarding.
- The ability to remain calm when dealing with urgent issues or difficult conversations.
- The flexibility to participate in an Out-of-Hours/On-Call rota, including evenings, weekends and bank holidays as required.
Desirable Experience
Previous experience in domiciliary care scheduling, care coordination or health and social care administration would be highly desirable.
Experience using care planning, rostering or electronic monitoring systems such as PASS, Nourish, CarePlanner or similar would be beneficial.
Knowledge of CQC expectations, safeguarding procedures, medication timing, emergency care planning and community care delivery would also be advantageous.
Previous experience participating in an on-call service within domiciliary care would be beneficial but is not essential.
