Overview
We are seeking a dedicated and experienced Complex Care Coordinator to oversee and coordinate personalised care plans within a senior care environment. The ideal candidate will possess strong leadership and supervising skills, with a focus on delivering exceptional care tailored to individual needs. This role offers an opportunity to make a meaningful difference in the lives of our residents, ensuring their wellbeing and comfort are prioritised at all times. The position is paid and suitable for individuals with a passion for senior care and excellent organisational abilities.

Responsibilities

Develop, implement, and review personalised care plans in collaboration with residents, families, and healthcare professionals
Supervise and support care staff to ensure high standards of service delivery
Coordinate daily activities and ensure smooth communication between team members and external agencies
Monitor residents’ health and wellbeing, escalating concerns as necessary
Organise training sessions for staff to maintain compliance with care standards and regulations
Maintain accurate documentation of care activities and resident records using IT systems
Lead on the organisation of appointments, medication management, and other essential services
Foster positive relationships with residents, families, and external stakeholders to promote a supportive environment

Skills

Proven experience in supervising or managing within a care home or senior care setting
Strong leadership qualities with the ability to motivate and guide teams effectively
Excellent communication skills, both written and verbal, to liaise confidently with residents, families, and healthcare professionals
Competent in using IT systems for documentation, scheduling, and reporting purposes
Valid driving licence is desirable for travel between care facilities or appointments
Knowledge of developing and managing personalised care plans tailored to individual needs
Ability to demonstrate empathy, patience, and professionalism in all interactions
