Advanced Clinical Practitioner (ACP) – Care Home Support Team

We are seeking an enthusiastic ACP to join our Care Home Support Team within the developing Community Urgent Care Division. This is an exciting opportunity to make a real difference by supporting care home residents to remain safely in their usual place of residence, reducing avoidable hospital admissions and improving patient outcomes.
Detailed job description and main responsibilities
Working as part of a multidisciplinary team, you will provide advanced clinical assessment, diagnosis, treatment, prescribing, and care planning for patients with urgent healthcare needs.
You will collaborate closely with colleagues across primary, secondary, community, and social care services to deliver person-centred care and support timely hospital discharge where appropriate.
This role also includes Team Lead responsibilities, with protected time allocated for leadership and management activities.
A key focus will be proactive care planning, supporting patients with complex needs, and developing strong links with specialist and palliative care services. Key Highlights:
The post holder will provide specialised triage, assessment and input as an independent practitioner developing and leading relationships with members of the multi-professional team allowing for consultation and collaboration in a variety of health care settings including Manchester
Integrated Neighbourhood teams, Intermediate Care Services, Out of Hours Service, community beds and primary care.
This post holder will also function as the Team Lead, and will have protected time to undertake any necessary management tasks.
The focus will be to prevent unnecessary acute hospital admissions and
enable patients to remain safely and supported at home and prevent premature admissions into long term care facilities.
To assist patients to proactively manage and anticipate their future health needs and develop appropriate plans, in conjunction with their GP and other medical staff as well as Allied Health Professionals, Nursing and Social Care staff to maximise their independence and develop health
plans to manage future health problems and support self-care.
The post holder will lead in developing, delivering and evaluating high quality patient centred interventions in conjunction with the associated clinical teams to ensure this is incorporated within both the overarching Community Urgent Care Division and the Manchester Local Care Organisation’s objectives.
