Job Summary
We are seeking a dedicated and passionate Health and Wellbeing Coach for a 12-month fixed-term contract. The successful candidate will play a vital role in supporting individuals to achieve their health goals through personalised coaching, education, and behavioural change strategies. This position offers an excellent opportunity to contribute to public health initiatives and promote healthier lifestyles within the community. The role requires a proactive approach, excellent communication skills, and a strong foundation in health-related disciplines.

Responsibilities

Provide personalised health and wellbeing coaching to patients with long-term conditions or those at risk of developing health problems.
Support patients to set achievable goals and develop self-management strategies.
Use behaviour change techniques and motivational coaching approaches.
Work with patients individually or in groups, either face-to-face, by telephone, or virtually.
Signpost and connect patients to local community services and support networks.
Support patients to improve areas such as physical activity, healthy eating, weight management, smoking cessation, stress management, and overall wellbeing.
Work collaboratively with the wider PCN multidisciplinary team.
Maintain accurate and timely clinical records.
Contribute to service evaluation, data collection, and quality improvement activities.

Experience

Has experience of health coaching, wellbeing coaching, health promotion, or behaviour change interventions.
Possesses excellent communication and interpersonal skills.
Can motivate and empower people to make sustainable lifestyle changes.
Has experience working within healthcare, community, voluntary, or social care settings.
Is able to work independently and as part of a multidisciplinary team.
Has strong organisational and IT skills.

Pay: £27,898.00-£30,784.00 per year
