We have an exciting opportunity for a Community Outreach Support Worker to join our team in Ware. This is a varied, rewarding role combining hands-on support with community engagement and outreach.
About the Role
This is more than a support worker role—it’s an opportunity to actively shape lives and communities.
You’ll:
- Provide person-centred support to adults with learning disabilities, autism, mental health needs, and older people
- Help individuals build confidence, independence, and social connections
- Plan and deliver engaging activities and community experiences
- Work beyond the hub to connect people with local opportunities and services
- Actively build relationships with community groups, organisations, and partners
- Represent Guideposts in the local area and help grow our reach
You’ll spend most of your time working directly with people and in the community, with admin kept to essential record-keeping and safeguarding.
Key ResponsibilitiesSupport & Wellbeing
- Deliver tailored, person-centred support
- Promote independence, inclusion, and confidence
- Support group activities in the hub and local community
- Maintain strong communication with families and carers
Community Outreach
- Build partnerships with local organisations and groups
- Identify new opportunities for activities and referrals
- Represent Guideposts at events and community forums
Service Development
- Contribute ideas for new groups and activities
- Support the growth and sustainability of services

Essential Experience & Skills
- Experience in care, support, wellbeing, or community work
- Ability to build trust and positive relationships
- Comfortable working in community settings
- Basic IT skills for record-keeping
Desirable
- Experience in outreach or community development
- Experience supporting adults with additional needs
- Knowledge of local services and resources
Additional Requirements
- Enhanced DBS check
- Full UK driving licence and access to a vehicle (business insurance required)
- Willingness to travel locally
