Full job description

Cedar Oak Healthcare is a growing domiciliary care provider supporting adults in their own homes. We are looking for an organised, reliable and compassionate Care Coordinator to support the day-to-day running of our homecare service.

This is an important role within the office team. You will assist the Registered Manager with coordinating care delivery, producing staff and client rotas, supporting care workers, maintaining accurate records, and helping to ensure that care is delivered safely, on time and in line with each person’s care plan.

The ideal candidate will have experience in domiciliary care, strong communication skills, good attention to detail, and the ability to work calmly in a busy care environment.

Key Responsibilities

Prepare and manage staff and client rotas to ensure all care visits are covered safely and efficiently.
Ensure care workers are matched appropriately to service users based on skills, location, availability and individual care needs.
Provide clear written and verbal instructions to care staff regarding their schedules and care visits.
Answer telephone and email enquiries professionally, politely and promptly.
Maintain accurate care records, staff records and electronic records.
Support the Registered Manager with the day-to-day running of the service.
Monitor care delivery and report any concerns, missed calls, late visits or quality issues to the Registered Manager.
Support with care plan reviews, risk assessments and service user visits where required.
Review daily notes, medication records, finance records and other care documentation as directed.
Assist with staff recruitment, induction, training, supervision and spot checks.
Help ensure staff receive appropriate support, guidance and supervision.
Liaise professionally with service users, families, care workers, social workers, commissioners and healthcare professionals.
Support quality assurance processes, audits and action plans.
Report safeguarding concerns, incidents, accidents or changes in service user wellbeing promptly.
Work in line with company policies, CQC requirements, safeguarding procedures, GDPR and health and safety legislation.
Provide emergency cover or support with care delivery where required.
Maintain confidentiality and represent the company professionally at all times.

Essential Requirements

Right to work in the UK is required. Cedar Oak Healthcare is currently unable to offer visa sponsorship for this role. Applicants must already have the right to work in the UK.
Previous experience working in domiciliary care, homecare, supported living or a similar health and social care setting.
Experience as a senior carer, supervisor, care coordinator or similar role.
Good understanding of rota planning, care scheduling and safe staffing.
Strong communication skills, both written and verbal.
Ability to prioritise, organise workload and meet deadlines.
Ability to maintain accurate written and electronic records.
Good understanding of safeguarding, confidentiality, dignity, respect and person-centred care.
Good IT skills and confidence using office systems or care management software.
Reliable, professional and able to work well under pressure.
Full UK driving licence and access to a vehicle.
Willingness to complete an Enhanced DBS check.

Desirable Requirements

NVQ Level 3 in Health and Social Care, or equivalent.
Experience using digital care planning or rostering systems.
Experience completing spot checks, supervisions or care reviews.
Knowledge of CQC standards, MCA, health and safety, GDPR and risk management.
Experience working with local authority or private care packages.
