The Nurse Assessor plays a key role in delivering high-quality care and rehabilitation support for patients following acute hospital admission within our Discharge to Assess beds. The role involves carrying out comprehensive health assessments, evaluating patient progress, undertaking risk assessments, and developing holistic care plans to ensure patients receive appropriate and timely support. The post holder uses expert clinical knowledge to complete Continuing Healthcare (CHC) and Fast Track assessments in line with Department of Health guidance, while promoting patient safety, dignity, and best interests.
Working closely with the multidisciplinary team (MDT), the Nurse Assessor coordinates complex discharge pathways from admission through to discharge. The role requires strong case management skills, sound clinical judgement, and the ability to work both independently and collaboratively to achieve positive patient outcomes.
The post holder also provides professional advice and support to colleagues, junior staff, and learners, contributing to the delivery of high standards of care across the service. They are responsible for maintaining accurate records, ensuring legal and professional accountability, and keeping their clinical skills and knowledge up to date. 1. Provide specialist nursing assessment, treatment, care planning and case management for patients within the service, managing a defined caseload autonomously.
2. Develop, implement and evaluate individualised care programmes and care packages to achieve positive patient outcomes.
3. Provide specialist clinical advice, guidance and support to patients, families, carers and multidisciplinary colleagues.
4. Undertake comprehensive assessments, including risk assessments, and formulate appropriate treatment and management plans.
5. Monitor, review and evaluate patient progress, adapting care plans and interventions as required.
6. Safeguard vulnerable individuals by identifying, reporting and managing safeguarding concerns in accordance with local and national policies.
7. Lead and contribute to multidisciplinary discussions, providing specialist clinical expertise to support decision-making and patient care.
8. Support continuity of care by coordinating services and working collaboratively with health, social care and independent sector providers.
9. Contribute to service development, quality improvement initiatives, clinical audit and implementation of evidence-based practice.
