Within Hayes Wick PCN are seeking an enthusiastic PCN pharmacist to support in improving medicines management and medicines optimisation across the PCN. We are looking for someone with a passion in long term condition management, in particular in one of the following areas; respiratory or diabetes. Ideally, the post holder will hold a prescribing qualification although this is not essential.
Duties and Responsibilities
Patient Services
- To provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients.
- To carry out telephone consultations and where clinically indicated face to face consultations and to sign own prescriptions if a prescriber
- To provide support to medication related query clinics
- To provide structured medication review clinics to those patients as identified by the National DES framework.
- To provide routine chronic disease reviews for patients.
- To support the practices with reconciling hospital correspondence such as discharge summaries and clinic letters.
- To liaise with local community pharmacy teams to support with medication related queries and medicines reconciliation.
- To liaise with acute and other care providers to support patient care.
- To provide medicines support and advice to the PCNs linked care homes
- To support with long term condition management
Budgetary
- To support with the implementation and achievement of medicines optimisation and QIPP targets as set by the local ICB.
- To work with the practices and the ICB team in addressing areas such as high cost drugs, hospital only drugs and specials.
- To support the practices in achieving their prescribing incentive/medicines management schemes.
Other
- To work as part of the multidisciplinary teams within the PCN practices and to support the other HCPs within the practices with medicines management
- To communicate and implement medication changes and alerts as advised by the medicines management team of the ICB or NICE guidelines or any other appropriate professional body.
- To engage with and support the practices with achieving the Quality Outcomes Framework (QOF) targets.
- To develop medication protocols for managing clinical conditions.
- To support with the implementation of repeat prescribing policies
- To support with clinical audit in the practices
Education and Training
- To participate in teaching and training of members of the pharmacy team and the wider PCN team
- To participate in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
- To participate in any relevant systems training necessary to fulfil the role e.g. PCPEP
- If not already, to work towards becoming an independent prescriber
- Take responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.

- Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards and reporting potential risks identified.

Working Conditions
- The post-holder will be required to use a computer regularly during the normal course of their duties.

Person Specification
Qualifications
Essential
- Recognised pharmacist qualification. Masters degree in pharmacy (MPharm) or equivalent.
- Registered with the General Pharmaceutical Council
Desirable
- Completed the CPPE Clinical Pharmacists in General Practice Education Programme or have sufficient experience to be exempt from the PCPEP pathway.
- Completion of/or working towards independent prescriber qualification.
- Specialist knowledge acquired through post-graduate diploma level or equivalent training/experience
ExperienceEssential
- Minimum of 3 years post-qualification experience.
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
Desirable
- Experience of working in a GP practice.
- Experience in the use of GP clinical systems especially EMIS.

- Ability to plan, manage, monitor, advise and review general medicine optimisation issues.
- Ability to work to policies and procedures including confidentiality, safeguarding, lone working, information governance and health and safety.
- Ability to support and mentor staff.
- Audit and reporting experience. IT skills e.g. Microsoft Word, Excel etc. Experience training other staff members.
Desirable
- Experience in the use of EMIS, Eclipse and Docman.
- Knowledge of interpreting and understanding prescribing budgets.
