West Berkshire Council is seeking a dedicated and compassionate Care Practitioner to join our Adult Social Care Service. As a Care Practitioner, you will play a vital role in supporting individuals to achieve their maximum independence and enhance their overall well-being.

Role and Responsibilities

As a Care Practitioner, your key responsibilities will include:

To work with and alongside individuals to assess, plan, implement and review how the individual’s identified care and support needs will be met.
To support individuals to achieve their identified goals and aspirations.
To undertake regular reviews of individual’s care and support needs.
To work with other professionals and agencies to ensure coordinated and holistic care delivery.
To provide information, advice, and guidance to individuals and their families.
To promote independence and empower individuals to make informed choices.
To accurately record and maintain information regarding individual’s progress and outcomes.

Skills and Attributes

The ideal candidate will possess the following skills and attributes:

Experience working in a care or support role within an adult social care setting.
Sound knowledge of care principles, practices, and relevant legislation.
Excellent communication and interpersonal skills, with the ability to build positive relationships with individuals, colleagues, and external stakeholders.
Strong assessment and care planning skills.
Ability to work independently, as well as part of a multidisciplinary team.
Organisational skills, with the ability to manage competing priorities and deliver within deadlines.
Empathy, compassion, and a genuine desire to support and empower individuals.
