Project Worker – Job Description

The Job

The Project Worker will support and enable the Centre Manager by participating in a planned programme of stimulating activities in relation to the needs identified for the group., developing a need led service.

Support vulnerable older people to have an active part in their community.

The Post is split into 3 components,

Dementia Café, Kit Club and Outreach service and will be a mix of in house and out in the community working.

Hours of the post will normally be 25 hours per week, over 5 days,

Monday-Friday.

Project Worker will be required to undertake flexible working times to suit the needs of the service.

The Tasks

Project Worker will:

1. Assist in Development of Services.

2. To work either independently or part of a team offering support and assistance to individual older venerable people who are socially isolated and lonely. In accordance with their identified needs and expressed wishes enabling them to build on confidence and self-esteem. This enabling them to remain as independent as possible.

3. Have an understanding of group/individual challenges and concerns.

4. Lease with other agencies to co-ordinate services

5. To develop activities, in full consultation with members and provide and participate in a stimulating & creative programme of activities, including arranging information sessions and where relevant speakers.

6. Duties to include but not unlimited to, Prescription collecting, shopping, befriending, providing support in individuals homes.

7. Check development of members with the regard to their goals.

8. Maintain appropriate records ensuring confidentiality at all times.

9. Prepare reports on service for Manager.

10. Maintain contact with relatives/carers and essential support network as needs demands.

11. Undertake work load in respect of all services including domestic tasks.

12. If required assist in Day Care area

13. Work extra hours in the centre if/when required

14. Assist Centre Manager in ensuring that the requirements of the Health & Safety Act and any other statuary requirements, so far as they affect the Centre are adhered to.

15. To work in accordance with Abbeyview Day Centres Policies & Procedures, ensuring confidentiality at all times and adhering to the General Data Protection Regulations May 2018.

16. Carry out any appropriate task delegated by the Centre Manager

17. The Project Worker will attend appropriate training courses

Line Managers Responsibility

The Project Worker will be responsible to the Centre Manager

Any queries about the Project Worker ability to carry out the tasks outlined, will be referred to the Centre Manager and Management Committee.
