Hours available. Hours to be agreed on appointment- up to 60 hours available.
The Hub will act as a single point of access for all referrals, enquiries, and contacts related to Community Therapy/Rehab Centre and District Nursing and other community services, making it easier for patients and professionals to connect with the right care quickly and efficiently.
We are seeking enthusiastic, adaptable individuals to join us in developing and delivering this innovative service. You will be part of a collaborative team dedicated to creating a seamless and supportive pathway for our community services.
