Post Overview – Care Navigator Spa Surgery

Spa Surgery is looking for a warm, capable and enthusiastic Care Navigator to join our friendly, high-achieving team in central Harrogate.

Care Navigators are at the heart of our practice. You will oftern be the first person a patient speaks to ad what you do next really matters. By listening carefully, asking the right questions and using your judgement, you will help each patient find the most appropriate care and ensure important information reaches the right people.

No two conversations are quite the same. Ome moment you might be reasurring someone who is worried and unsure what to do; the next, arranging support for vulnerable patient, solving an unexpected problem or working with our clinical team to keep patient care moving.

It is a busy and varied role for someone who enjoys thinking on their feet, getting things organised and finding a way forward. You will go home knowing that your work has made a genuine difference, not only to our patients, but to the whole practice team.

You will not be epected to know everything from day one. We will provide full training, clear procedures and support from an experienced team, alongside a weekly huddle where we share information, learn together and keep everyone connected.

About Spa Surgery - Spa Surgery is a friendly and ambitious GP practice caring for a varied patient population. Alongside our general practice services, we provide care to:

People living in specialist homes for those with learning dissabilities and long-term mental health conditions.
residents of nursing and residential homes
People experiencing homelessness through our work with Springboard, a local day centre

We are proud of the relationships we build with our patients and of finding practical ways to help people access the care they need.

What you will be doing:

As a Care Navigator you will:

Welcoming patients contacting us in person or by telephone.
Listen to patients' needs and help them access the most appropriate member of the practice team or external service.
Support vulnerable and higher-risk patients, including people with learning disabilities, dementia, mental health needs or long-term conditions
Respond to a wide variety of questions and requests, using your judgement to problem-solve, signpost or escalate concerns
Arrange and manage appointments for defined groups of patients
Process prescriptions, clinical correspondence, pathology results and administrative tasks accurately and promptly
Handle patient information safely and confidentially
Work closely with our clinical, pharmacy and administration teams
Participate in practice training, development sessions and team meetings

The role also includes practical reception and administrative responsibilities such as processing changes to patient details, monitoring emails, handling samples, issuing approved documents and supporting the wider administration workload.

Full training will be provided in our systems, procedures and care navigation pathways.

Who we are looking for

You may already have experience in healthcare, care, customer service, hospitality, administration or another busy public-facing environment.

More important to us is that you are:

Warm, approachable and genuinely interested in helping people
A confident communicator who can listen as well as talk
Calm and professional when someone is worried, frustrated or upset
Comfortable making decisions within clear procedures
Naturally organised, accurate and able to manage competing priorities
Confident using computers and learning new systems
Discreet and trustworthy when handling confidential information
Positive, dependable and willing to contribute to the team
Able to recognise when something needs to be escalated

You will need to be comfortable working in a busy environment where priorities can change quickly. In return, you will become part of a knowledgeable and supportive team and develop valuable experience within NHS primary care.

What we offer

Hourly rate increasing from £12.71 to £13.25 following successful completion of a statutory 4 month probation period

The successful applicant will be required to complete satisfactory DBS and right-to-work checks

Hours required: 27 per week - Mon & Tues 12pm -6pm, Thurs & Fri 10am-6pm

Hours required: 37.5 per week - Tues-Fri 8am - 6pm

Job Type: Permanent

Pay: From £12.71 per hour
