Salary: £27,000 – £28,500 per annum
Job Type: Full-time
Schedule: Monday to Friday, with weekend availability and rotated on-call duties as required

Job Summary

We are seeking an organised, proactive and reliable Care Coordinator to support the effective delivery of high-quality domiciliary care services.

The successful candidate will be responsible for coordinating care schedules, allocating staff appropriately, responding to changes in daily rotas, supporting service continuity, and maintaining clear communication with care staff, service users and their families. This role requires someone who can remain calm under pressure, manage competing priorities and ensure care is delivered safely, consistently and in line with company policies and regulatory standards.

The role will also include rotational on-call duties to support out-of-hours service delivery, respond to urgent staffing or service user issues, and escalate concerns appropriately.

Key Responsibilities

As Care Coordinator, you will:

Develop, manage and update staff rotas to ensure service users receive safe and consistent care.
Allocate care staff effectively, taking into account service user needs, staff availability, location, skills and continuity of care.
Monitor daily schedules and respond promptly to sickness, absence, emergencies, late calls, missed calls and changes in care needs.
Participate in the rotated on-call system, responding to urgent out-of-hours calls and escalating issues where required.
Work collaboratively with management and care teams to resolve staffing gaps and coverage issues.
Communicate professionally with care staff, service users, families and other relevant professionals.
Ensure staff are informed of rota changes, call requirements, service user updates and any urgent instructions.
Maintain accurate records on the company’s care management system.
Monitor call delivery and follow up on concerns such as late log-ins, missed visits, incidents, accidents or changes in service user presentation.
Escalate safeguarding, medication, staffing, conduct or service delivery concerns in line with company procedures.
Support care staff with day-to-day queries and provide practical coordination support where required.
Liaise with service users and families to support continuity of care and respond to concerns professionally.
Assist with general office, administrative and operational duties connected to the smooth running of the care service.
Support compliance with company policies, safeguarding expectations, CQC standards and domiciliary care best practice.
Carry out any other reasonable duties related to the role and the needs of the service.

Skills and Competencies

The ideal candidate will have:

Strong organisational and time management skills.
Excellent communication and interpersonal skills.
Ability to work under pressure and manage changing priorities.
Good problem-solving skills and a solution-focused approach.
Confidence using care management software and digital systems.
Good understanding of domiciliary care operations.
Awareness of safeguarding, confidentiality and professional boundaries.
Ability to make sound decisions, especially during urgent or out-of-hours situations.
Ability to work well as part of a team and communicate effectively with care staff and management.
Flexibility to support service needs, including participation in the on-call rota.

Requirements

Previous experience in care coordination, rota management or a similar role is preferred.
Experience within domiciliary care or health and social care is desirable.
Relevant Health and Social Care qualification, Level 3 or above, is preferred.
Good knowledge of CQC expectations and care compliance standards.
Full UK driving licence and access to a vehicle is essential.
Must be reliable, professional and able to work flexibly according to business needs.

About the Role

This is a key role within the care operations team. The Care Coordinator will help ensure that care calls are covered, staff are supported, and service users receive the right care at the right time. We are looking for someone who is organised, confident, professional and committed to supporting high standards of care delivery.

We are unable to offer sponsorship for this role at the moment.

Pay: £27,000.00-£28,500.00 per year
