Main Job Purpose
· In accordance with the aims of the national Changing Futures programme, provide long term, personalised support and case coordination to adults experiencing severe multiple disadvantage (complex interplay of mental health, substance misuse, homelessness and offending) in Nottingham city.
· Encouraging engagement from those who typically do not engage with services, enabling them to better manage their lives through a multi-disciplinary and person-centred approach.
· Use an understanding of how Black, Asian and other minority ethnic groups experience severe multiple disadvantages to connect with individuals and provide authentically personalised approaches to support.
- To contribute information and learning to the national Changing Futures programme, to bring about positive systemic change and improve services for people who experience Severe Multiple Disadvantage within the ethnic minority communities.
· Use an understanding and experience of the diverse values and cultures of people from different ethnicities in order to connect with individuals and provide authentically personalised approaches to support.
Main Job Duties and Responsibilities
· Provide direct case support to individuals with a combination of severe multiple disadvantage (homelessness, substance misuse, mental health and offending) as experienced by individuals from ethnic minorities.
· Provide long term, personalised support and case coordination to adults experiencing severe multiple disadvantages.
· Encouraging engagement from those who typically do not engage with services, enabling them to better manage their lives through a multi-disciplinary and person-centred approach.
· Provide direct 1-1 holistic needs assessment and case support to individuals with a combination of severe multiple disadvantage (homelessness, substance misuse, mental health and offending) as experienced by people from ethnic minorities.
· Assisting individuals on their case load with all aspects of creating and support to implement relevant Personal development and support plans, this includes:
- Conduct an individual needs assessment for all survivors into the project to capture housing and support needs, health and wellbeing, risks, strengths and preferences, and any other information relevant to the delivery of the service for each person accessing the service;
- Develop a focused, co-produced action plan for each person accessing the service to set out steps to, to maintain safety and to improve health and wellbeing;
-Liaising with the Changing Futures Programme operation team and a wide range of statutory and non-statutory agencies in order to ensure holistic assessment of need and referral or signposting to appropriate services.
· Understand and enact processes and pathways in relation to safeguarding and Domestic/Sexual Violence and Abuse (DSVA).
· All aspects of preparing and implementing service user risk assessments and risk management strategies.
· Responding to referrals by carrying out initial assessments to determine eligibility for the service and support referring agencies by supporting them to complete referral paperwork where needed.
· Versatile partnership working with other agencies to ensure that appropriate and prompt assessment is carried out.
· Support partner agencies to deliver personalised responses to female beneficiaries who experience SMD.
· To provide a close working link to specialist agencies who work with ethnic minorities, including forging new and improved relationships with wider agencies focused on the needs of people experiencing SMD.
· Develop effective relationships with partner agencies involved in the delivery of the Changing Futures Programme, including workers in embedded posts in statutory agencies.
Ensure
Work with specialist and wider partners who offer and provide support to address complex needs including mental health, substance misuse and/or alcohol misuse and offending, such as Nottingham Recovery Network, Adult Social Care services, GP’s, Hospital Discharge Teams, Probation Services and specialist services for refugees, sex working offending, etc. where conducive to the aims and outcomes expected of the service
· Support partner agencies to deliver personalised and culturally competent responses to beneficiaries from ethnic minorities.
· Recognise the role of language barriers, isolation from the community and family and racial discrimination for people from ethnic minorities.
· To identify, refer and/or signpost to appropriate agencies/service, including working with those agencies to deliver a personalised response.
· Work with statutory and non-statutory agencies to refer, signpost and support access to services. This includes participating in multidisciplinary meetings with other agencies, including making referrals, attendance and contributing to care planning in order to co-ordinate a beneficiary’s network of support.
· Adhering to the principles of clinical governance by keeping timely and accurate records and attending regular training.
· Promoting the service, its ethos and benefits to potential beneficiaries, other professionals and agencies, and the wider public.
GENERAL RESPONSIBILITIES

· Ability to understand and take action on race issues/inequality. Supporting Changing Futures in its aim to understand and provide appropriate culturally sensitive support to beneficiaries.

BREADTH OF THE ROLE
The Navigator performing this role will be expected to:
· Manage a case load of beneficiaries facing Severe Multiple Disadvantage and they will need to effectively plan and manage their workload according to the needs of those beneficiaries.
· Attend and contribute to multi-disciplinary meetings for each beneficiary, in order to effectively coordinate a beneficiary’s network of support, as well as maximise communication and ease of information sharing. These meetings and any actions arising will also contribute towards a beneficiary’s support plan.
· Take flexible and solution focused approach, that will sometimes require the Navigator to respond to an immediate need, within professional boundaries.
· Undertake a large amount of work either in the community or in the homes of beneficiaries, in line with the outreach model operated by the service. The Navigator will adhere to all organisational and service lone working procedures.
· Have an awareness and understanding of the aims of the national Changing Futures Programme and will actively contribute towards its agenda of system change, as well as assisting with robust data collection.
· Be able to work in a challenging environment with other agencies/ services that may at times have differing priorities.
· Work to understand people in the context of past trauma, including the skills to gather. information sensitively and in a trauma informed way during the initial assessment period.
· Reflect on practice and actively use supervision.
· Respond calmly and constructively to challenging situations.
· Participate actively as part of the broader, multi-agency Navigator team deployed across the Changing Futures partnership.
· Liaise closely with and advise when required Changing Futures Peer Mentors assigned to support relevant beneficiaries.
Monday to Friday, 9-5pm.

Effective partnership working is fundamental to the successful delivery of the Specialist Navigator. This includes between the partners directly involved in the delivery of the service as well as with other agencies who can provide support. The service will need to liaise regularly with a multitude of partners in order to support the individual in progressing their personalised support plan. This includes agencies focused on:
