We are now seeking an experienced Care Coordinator to join our dedicated team and help ensure the smooth, efficient delivery of community-based homecare services.
The Role
As a Care Coordinator, you will support the Registered Homecare Manager and oversee the daily operations of our care coordination team. You will ensure that all care packages are delivered effectively, staff rotas are maintained, and the highest standards of care and compliance are achieved.
This role requires a highly organised, proactive individual with strong leadership skills and experience in domiciliary care coordination.
Key Responsibilities
Service Coordination
- Support the Registered Homecare Manager in delivering safe, person-centred care services within the community.
- Ensure care rotas are completed at least 4 weeks in advance and communicated to staff.
- Maintain and update the rostering system with staff availability and service user requirements.
- Ensure 100% compliance with call monitoring and visit logging systems.
- Maintain accurate client records and ensure care notes are updated regularly.
Team Leadership
- Oversee and support a team of Care Coordinators.
- Conduct performance monitoring, supervision, and appraisals of direct reporting staff.
- Ensure staff attend mandatory training and remain compliant with regulatory requirements.
- Promote staff wellbeing and ensure safe working practices are maintained.
Operational Duties
- Work alongside Risk Assessment Officers to implement personalised care packages.
- Coordinate staffing to ensure all care calls are covered.
- Provide care support when required due to staff shortages.
- Support with unallocated care calls when all other options are exhausted.
- Participate in the on-call rota (1 weekend in 4 and 1 night per week).
Compliance & Quality
- Maintain 100% HCA compliance across the service.
- Ensure adherence to the organisation’s Code of Conduct and staff handbook.
- Participate in investigations relating to incidents, complaints, or concerns.
- Ensure compliance with CQC and commissioner standards.
- Support quality improvement through learning from incidents and feedback.
Service Development
- Support the Registered Manager in identifying opportunities for service growth.
- Assist with sourcing new care packages and expanding the department.
Requirements
Essential
- NVQ Level 3 in Health & Social Care
- Minimum 3 years’ experience in homecare
- Experience in a care coordination or senior coordination role
- Strong IT skills, including Microsoft Excel and care management systems
- Experience using Access People Planner
- Excellent organisational and time management skills
- Ability to manage multiple priorities in a fast-paced environment
- Full UK Driving Licence
Desirable
- Experience supervising or mentoring care coordination teams
- Understanding of CQC standards and regulatory requirements
- Experience managing large rota schedules and workforce planning

Working Pattern
- Monday – Friday
- Day shifts/ Weekends
- On-call responsibilities: 1 weekend in 4 and 1 night per week Monday to Friday.
