Exciting opportunity for a Full-Time Patient Discharge Coordinator to join our team at HTG-UK based at Royal Cornwall Hospital

What Can You Expect?

You will be joining us in the role of Patient Discharge Coordinator, based at the Royal Cornwall Hospital and occasionally our base in Redruth. This is a full-time, permanent position (37.5 hours per week, Monday to Friday).

As the Patient Discharge Coordinator, you will be responsible for ensuring the smooth and timely discharge of patients from hospital or the completion of treatment by proactively liaising with all key stakeholders. This will include working closely with patients, healthcare professionals, and HTG staff to coordinate and deliver efficient, effective, and patient-centred transport solutions that support safe and timely transitions of care.

Summary Of Main Duties Include

Coordinate timely patient discharges through effective communication with hospital teams and HTG staff
Liaise with ward and discharge teams to support smooth patient flow
Book and manage sameday patient transport requests
Update patient status accurately using HTG systems
Work proactively to identify and resolve discharge or transport delays
Liaise with Transport Control Centres to support timely vehicle dispatch
Support a positive patient experience within the discharge setting
Use HTG resources efficiently in line with operational and contractual requirements

What Were Looking For:

Experience in hospital discharge, healthcare coordination, patient transport, or similar settings.
Knowledge of NHS discharge processes and the role of transport in patient flow.
Understanding of how timely discharges support bed management and clinical priorities.
Comfortable working under pressure in fastpaced hospital or control environments.
Ability to prioritise patient journeys and manage discharges in real time.
Competent with Microsoft Office (especially Outlook and Excel) and quick to learn internal systems.
Strong attention to detail when recording patient information and updating systems.
Proactive in identifying discharge delays and escalating issues appropriately.
Calm, professional approach to problemsolving and sensitive situations.
