Lancashire and South Cumbria NHS Foundation Trust (LSCFT) are seeking 2 x skilled Band 6 Triage Clinical Coordinators to join our Care Connexions team. This is an exciting opportunity to work at the heart of integrated care, playing a key role in the clinical triage, prioritisation, allocation, and coordination of referrals across a range of urgent and community-based services.
This is a rewarding and fast-paced role where no two days are the same. You will be instrumental in supporting patients to remain safely in their own homes, reducing avoidable hospital admissions, facilitating seamless care coordination, and improving patient outcomes. We are looking for clinicians with experience of working across health and social care settings, excellent communication and decision-making skills, and a strong understanding of integrated community services and admission avoidance pathways.
