Main duties, tasks & skills required
The post holder will work autonomously within their professional scope, taking responsibility for all aspect of community health care delivery and the safe day to day running of the service. They will line manage a team of staff, support the management of an active caseload, and oversee staff rostering. They will also participate in the triage rota, carrying delegated authority to admit, refer and discharge patients. The Community Charge Nurse will be responsible for incident management, including reviewing incident forms and ensuring appropriate follow up and learning
They will actively contribute to the Primary Care Network (PCN), involve the wider MDT where appropriate, and delegate tasks to ensure safe and effective care. Th role includes providing clinical supervision to new and existing staff, working within legal and ethical frameworks, and adhering to organisational policies. The post holder will also support quality improvement by identifying opportunities for service development within safe and appropriate boundaries
