As a General Practice Nurse at Bracondale Medical Centre, you will be responsible for delivering excellent evidence-based nursing care such as chronic disease management and health promotion. You will contribute to the smooth running of the nursing team and patient experience.

Main duties of the job
Responsible for the delivery of general practice nursing services, working as part of the practice multidisciplinary team. This includes identifying, managing and supporting patient at risk of developing long term conditions, routine nursing care in line with evidence-based practice and adhering to national, local and practice guidance. Providing childhood, adult and travel vaccinations, women’s and men’s health promotion, contraceptive services and cytology.
Collaborating with the practice team including the lead nurse, Health Care Support Workers, GPs, Practice manager and administrative staff.

The practice is dedicated to providing exceptional health care services with a personalised touch, prioritising the physical, emotional and mental health care of every individual. The practice has a patient list of 5800 with a dedicated team of GP partners, salaried GPs, Lead Nurse, Health care Support Workers and other clinicians from our primary care team. We are a GP training practice and support having nursing students.

1. Job Purpose
To work as part of a friendly and supportive multidisciplinary team at Bracondale Medical Centre, providing high‑quality, evidence‑based nursing care to the practice population. As a General Practice Nurse (GPN) you will deliver a wide range of nursing services, including long‑term condition management, immunisations, health promotion and screening, in line with national and local guidance, practice policies, local and nationally contracts and QOF requirements.

- Practice Manager and administrative / reception staff
- PCN staff (e.g. Care Coordinator, Physiotherapist, Clinical Pharmacists, Social Prescribers, Mental Health Practitioner)
- Community nursing teams, health visitors, midwives, secondary care and other external providers
3. Main Duties and Responsibilities
General Clinical Duties
- Provide evidence‑based, person‑centred nursing care to patients across the life course.
- Assess, plan, implement and evaluate nursing care for patients, documenting clearly and contemporaneously.
- Recognise and act on red flag symptoms and concerns, escalating to a GP or senior clinician as appropriate.
- Work within NMC Code, practice policies, local and national clinical guidelines.
Long‑Term Condition Management
- Provide routine and recall reviews for patients with long‑term conditions, for example:
- Diabetes
- Chronic obstructive pulmonary disease (COPD)
- Asthma
- Hypertension
- Coronary heart disease (CHD) / cardiovascular disease (CVD)
- Heart failure
- Stroke / TIA
- Chronic kidney disease (CKD)
- Undertake structured reviews including:
- History taking and symptom assessment
- Physical observations (BP, pulse, weight, BMI, peak flow, oxygen saturation, etc.)
- Foot checks for people with diabetes
- Inhaler technique checks and optimisation
- Lifestyle assessment (smoking, alcohol, diet, exercise)
- Implement and monitor individualised care plans, in line with NICE guidance and local pathways.
- Provide self‑management support and education and referrals to public health schemes and relevant community teams.
- Work with GPs and clinical pharmacists on medication reviews within scope of practice
- Contribute to achievement of QOF indicators related to long‑term conditions.
Immunisations and Vaccinations
- Deliver the full range of childhood immunisations in line with the UK Immunisation Schedule and local PGDs / PSDs.
- Provide adult immunisations, including:
- Seasonal influenza
- Pneumococcal
- RSV
- Shingles
- Pertussis in pregnancy
- Travel vaccinations - NHS provided vaccinations
- Other vaccines as per national guidance and local commissioning
- Undertake pre‑ and post‑vaccination counselling, including contraindications, side effects and consent.
- Maintain accurate immunisation records and ensure timely data entry and coding.
- Participate in vaccine ordering, stock rotation and fridge checks, cold chain monitoring and incident reporting.
Cervical Screening and Women’s / Men’s health
- Perform cervical screening in line with the NHS Cervical Screening Programme and practice protocol.
- Provide contraception and sexual health advice within competence, including:
- Contraceptive counselling and follow‑up
- STI prevention advice and signposting for testing / treatment
- Support women’s health clinics (e.g. HRT reviews, menopause support) in line with competence
- Support men’s health initiatives (e.g. health checks, PSA discussions within protocol).
Health Promotion and Screening
- Deliver and promote NHS Health Checks and other health screening programmes as commissioned.
- Provide lifestyle advice on:
- Smoking cessation and referral to stop smoking service
- Weight management and healthy eating and referral to appropriate services
- Physical activity and referral to exercise on prescription scheme
- Alcohol reduction and substance misuse signposting
- Support practice and PCN initiatives to reduce health inequalities and improve uptake of screening and vaccination in underserved groups.
- Work with Social Prescribing Link Workers and other PCN roles to support holistic, personalised care.
Infection Prevention and Control (IPC)
- Adhere to practice IPC policies, including hand hygiene, PPE, sharps safety and waste segregation.
- Ensure clinical rooms and equipment are cleaned and maintained in line with protocols.
- Participate in audits (e.g. hand hygiene, sharps, cleaning records) and implement agreed actions.
- Support the practice’s IPC lead in maintaining CQC‑compliant standards
Safeguarding
- Be aware of and comply with safeguarding children and adults’ policies and procedures.
- Recognise signs of abuse, neglect or exploitation and escalate concerns promptly to the safeguarding lead / GP.
- Attend mandatory safeguarding training at the level required for the role.
- Document safeguarding concerns accurately and confidentially.
Medicines Management (within competence)
- Administer medicines safely under PGDs, PSDs or prescriptions, in line with NMC standards.
- Check allergies, interactions (where appropriate) and contraindications before administration.
- Maintain accurate records of all medicines administered.
- Support safe storage, stock control and disposal of medicines in clinical areas.
- Work with clinical pharmacists and GPs on medicines optimisation projects where appropriate.
Information Management and Record‑Keeping
- Maintain accurate,contemporaneous and legible clinical records in the practice clinical system.
- Use appropriate SNOMED / Read codes to support QOF and other reporting requirements.
- Ensure results, referrals and follow‑up actions are clearly documented and actioned.
- Respect confidentiality and comply with data protection legislation and practice policies.
Teamwork and Communication
- Work collaboratively with the practice and PCN multidisciplinary team.
- Attend and contribute to practice meetings, nursing team meetings, clinical meetings and significant event reviews.
- Communicate effectively and professionally with patients, carers and colleagues, including when dealing with complaints or difficult situations.
- Provide clinical advice and support to HCSW’s and other staff within scope of competence.
- Act as a chaperone when required, in line with practice policy.
Quality Improvement, Governance and CQC
- Participate in clinical audit, quality improvement projects and service development, particularly in areas such as long‑term conditions, vaccination and screening.
- Contribute to achievement of QOF and local enhanced services targets.
- Assist in developing, reviewing and implementing clinical protocols and guidelines.
- Support preparation for CQC inspections, providing evidence of nursing activity, audits and outcomes
- Report and help investigate incidents, near misses and complaints, and implement learning where relevant.
Education, Training and Professional Development
- Maintain NMC registration and revalidation, keeping an up‑to‑date professional portfolio.
- Take responsibility for own CPD, attending mandatory and role‑specific training as required.
- Participate in annual appraisal and agree a personal development plan.
- Support the development of others, e.g. mentoring student nurses, HCSWs or new staff if appropriate.
- Keep up to date with NHS England, NICE and local ICB guidance relevant to general practice nursing.

Qualifications / Registration
- Registered Nurse (Adult) with current NMC registration.
- Evidence of recent CPD.
- General Practice Nursing experience
- Additional training in chronic disease management, cervical screening, immunisations, contraception, etc.
Experience
- Experience of working in primary care / general practice.
- Experience in managing long‑term conditions.
- Experience of working in a multidisciplinary team.
Skills and Knowledge
- Strong clinical assessment and decision‑making skills within scope of practice.
- Competence in core GPN skills : immunisations, cervical screening, LTC reviews.
- Good IT skills and familiarity with electronic patient records / EMIS
