We are looking for an experienced and motivated Advanced Practitioner to join our Mental Health and Specialist Services, working across community and specialist teams to drive high-quality, person-centred practice.
This is a fantastic opportunity to step into a leadership role, supporting managers, shaping practice, and making a real difference to people with complex mental health and care needs.
About the Role
Working across adults and older adults’ services, you will:
- Deputise for the Team Manager and support operational leadership
- Lead on quality assurance, performance, and practice improvement
- Coordinate and support safeguarding and complex casework
- Provide line management to a small group of Social Care Coordinators and Wellbeing Officers
- Champion strengths-based, trauma-informed and relationship-based practice
- Mentor, coach and support colleagues to deliver consistent, high-quality outcomes
You will play a key role in embedding Care Act principles, ensuring practice focuses on wellbeing, personalisation, and meaningful outcomes.

What We’re Looking For
You will bring:
- Significant experience in adult social care, including complex casework and safeguarding
- Strong working knowledge of the Care Act, Mental Capacity Act, and Mental Health legislation
- A passion for practice development, mentoring, and quality improvement
- Confidence to lead, challenge and support others in a fast-paced environment
- Experience of risk assessment, strengths-based practice and multi-agency working
