Full job description

Strayside Health, a new GP practice formed by the merger of East Parade and Park Parade Surgeries in Harrogate, is excited to invite applications for the role of Care Coordinator. This is a unique opportunity to join a dynamic and growing practice dedicated to delivering high-quality patient care. As a Care Coordinator Team member, you're often the first person a patient meets when they come to the surgery. You'll book patients in for appointments, enter patients details onto our IT system and direct patients where to go. As well as dealing with patients face-to-face, our patient services team answer the phones, directing calls to other staff, book appointments, answer queries. In addition, there will be some admin duties. Getting to support both patients and the medical team is rewarding and fulfilling. What you do will mean a lot to our patients; you'll put their mind at ease, often you'll make their day! No two days are the same. You'll always be learning, solving problems and taking the pressure off the medical team. It can be stressful at times. There's never a dull moment! At times patients will be upset, frustrated or angry. You will need to listen, try to understand and empathise. It helps if you can put yourselves in their shoes. You will need to work with care and attention in a fast-paced environment and multitask even when under pressure. We will expect you to be professional; honest, reliable and respectful under pressure. Confidentiality is a must.You will often need to ask patients about their problem and summarise this accurately for the medical team. You may need to redirect patients elsewhere and do so with confidence and warmth. At times you have to think outside the box.You will need the following skills;

Ability to work under pressure to tight and often changing deadlines
Able to work on own initiative
Flexible approach to work and able to adapt to change and uncertainty
Ability to maintain confidentiality and trust
Commitment to continuing professional development
Good time keeping
Problem solving skills
Ability to analyse and interpret information
Working knowledge of Microsoft Office with intermediate keyboard skills
Clear communicator with good writing skills
Excellent organisational skills, able to plan and manage demanding workload with competing priorities
Ability to follow policies and procedures

The successful applicant must be able to work some 7.45 starts / 6.30pm finishes; cover for other team members' holiday will be required therefore the ability to be flexible is essential. The starting hourly rate is £12.71 and there will be a 6-month probationary period. After a satisfactory probationary period the hourly rate will then rise to £13.38.
