Full job description

The Medicine for Older People Department at East Lancashire Hospitals NHS Trust is looking to recruit a highly skilled and motivated Advanced Clinical Practitioner in Frailty. This is an exciting opportunity for a qualified ACP to join an expanding and motivated team and to be involved in the ongoing development our frailty services.

Our team is led by two Consultant ACPs, with support from Consultant Geriatricians, Specialty Doctors and Junior Clinical Fellows, nursing leaders, AHPs and a wide and far reaching MDT. The Medicine for Older People Department has led significant changes to our frailty services over the past year, resulting in a high performing seven-day frailty service. We hope to recruit an enthusiastic ACP to contribute to our continued improvement.

Undertake clinical and holistic assessment of frail elderly patients in the Older Persons Rapid Assessment Unit and across other clinical areas as needed

Provide guidance and expert advice to other members of staff to ensure safe and effective care for frail elderly patients
Act as a clinical leader within the trust
Support and guide the ongoing development of frailty services across ELHT
Work closely with the wider multi-disciplinary team to ensure a high standard of care
