Job Introduction

At Turning Point, we support people across the UK with substance use issues. Our Herefordshire (West Midlands) Recovery Service responds to the needs of people across the county, working in locations throughout Herefordshire to maximise the opportunities for service users.

This position is an exciting opportunity to improve the pathway for people from hospital to the community for those who use alcohol and drugs. The post will involve close partnership working with Alcohol Liaison nurse and wider hospital teams, to assess and screen service users, deliver advice and information and hold a case load of specific clients as they are discharged from the hospital. You will join a committed and friendly team at Turning Point Hereford who take a multi-disciplinary approach with support from clinical, psychosocial interventions staff, peer mentors and a strong network of partner agencies.

As a Hospital Liaison Worker we offer a starting salary £25.870 with annual pay progression up to £30,265.

Role Responsibility

To assist in delivering Turning Point’s Substance Use strategy by planning and providing high quality, innovative care which reflects our person-centred values and the high levels of ambition we have for the recovery of the individuals for whom we provide support.

Key Responsibilities:

To deliver Hospital in-reach to A&E departments, medical wards and GI wards.
To complete assessments, provide early engagement, advice & brief interventions, for those identified as having substance use support needs by Hospital staff and develop packages of care within the community
Support develop the understanding of hospital staff regarding substance use treatment options and dispelling stigma where it may arise
Contribute to the development of after care packages in conjunction with the Hospital Alcohol liaison Nurse prior to hospital discharge.
To contribute to the development of seamless care pathways for continuation of detoxification regimes where these are initiated in hospital and are appropriate to continue in the community.
To work with families / significant others of those identified as having drug and alcohol support needs upon release from hospital to maximise social recovery capital and to support families / significant others. To involve them in recovery plans where possible.
Work flexibly in a variety of settings to create care and support plans that meet the needs of the service and clients in accordance with the recovery agenda

The Ideal Candidate

You’ll have an understanding of substance and alcohol use and the challenges and opportunities for individuals on a recovery journey. Having experienced of working in a pressured hospital environment would be of great benefit but not essential. However, we also recognise that many people have transferrable skills from other backgrounds such as mental health, housing or other health and social care provision. The roles are varied, so flexibility, an ability to work dynamically on a one to one and group basis and knowledge of relapse and other barriers to recovery will be vital but we can help to shape these skills where needed. We also look for candidates with excellent communication skills and who share our values as an organisation.

Your ideas will be listened to and your suggestions implemented wherever possible. You will be supported in your role by extensive training to help develop your skills. A background in hospital care, primary care, or mental health would be advantageous.
