- The Spalding PCN has been formed due to NHS Legislation and consists of the following practices:
- Beechfield Medical Centre, Spalding
- Munro Medical Centre, Spalding
The two practices have worked together for a few years and have a dedicated team attached to them.
We are in the exciting position of adding new roles to compliment the current surgery teams – to work towards bringing the four surgeries together under the Spalding PCN, to give the patients a patient-centred care approach.

- Main Purpose of Role
- To be on the FCP roadmap for paramedics, or about to embark on the course.FCPs work at master’s level (level 7) in their clinical pillar of practice but have not yet reached an advanced level in all four pillars of practice to be verified at AP level across all four pillars. The clinician must typically have 3-5 years post preceptorship experience before starting primary care training to become an FCP. NB: Experienced band 6s are well placed to to be working towards FCP but do not have to progress to that level of practice.
- The main aim for this job is home visiting of housebound patients, response and triage of urgent primary care clinical issues. The person will need to provide a robuse assessment of patients, presenting with unscheduled care needs in a variety of settings at point of first contact. Treat, diagnose, discharge and refer patients safely based on assessment findings.
- The Paramedic will be required to work within the multi-disciplinary structure within the Spalding PCN. Work with the service on the day to day triage and support of patients with acute medical issues, so be aware of plans and methods regarding delivering these services.
- The Paramedic will be required to demonstrate compassionate care in their daily work. Post holders will also be required to understand and work in accordance with the NHS constitution and put the patient at the heart of their work.
- Key Role Requirements
- Level 7 requirement
- Provide a rapid response to urgent patients requests for home visits and review; perform a thorough and comprehensive patient assessment and detail a management plan to ensure that the individual needs of each patient are met within the individual practitioner’s scope of practice.
- Effectively document clinical encounters on the electronic patient record and effectively communicate these encounters to colleagues.
- Undertake home visits.
- Instruct and educate patients in preventative health care.
- Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes;
- Organise and when available undertake diagnostic tests as required, for example taking blood and other samples for analysis
- Independently assess needs for, and perform therapeutic procedures such as joint immobilisation, pharmacological intervention in the form of patient group directions for oral medications or injections and immunisations.
- Liase with other agencies/disciplines (primary general practice) involved in the provision of care and treatment of patients and promote and develop cross partnership working
- Maintain required standards of competence and qualifications through continuing professional development.
- Demonstrate an understanding and application of evidence based practice.
- Demonstrate innovative ways of meeting individual patient and population needs within the confines of the PCN policies.
- Demonstrate a high standard of clinical information collection and documentation, and a use of all guidance around the collection and storage of information under the existing local and national guidance.
- Identify and assist in the collection of data for audit and research purposes as required
- Be required to support and move patients under the manual handling rules and regulations and use appropriate moving and handling equipment
- General
- Demonstrate innovative ways of meeting individual patient and population needs within the confines of the PCN policies.Ensure that all activities are monitored and evaluated.
- Work in accordance with PCN policies, including equal opportunities policy and practice.
- Attend any training courses and supervision sessions as required.
- Work with due regard to the PCN Health and Safety Policy ensuring that all practices and procedures are undertaken with issued guidelines.
- Attend regular MDT Meetings organised by the PCN
- The post holder will ensure compliance with the requirements of Caldicott, the Data Protection and Human Rights Act and other relevant legislation and guidance.
- Promote client involvement in the management of the services
- Maintain administration systems and workspace used in a clean and tidy condition
- Work flexibly to meet the needs of clients
