We are looking for an enthusiastic Band 5 Physiotherapist to join our team
To provide a high-quality person-centred approach to care delivery which always considers people’s safety, privacy and dignity.
To undertake assessment of patients / service users care needs and develop programmes of care/Rehabilitation as part of the wider multi-disciplinary care team.
To provide advice to patients/ service users and carers regarding their care.
To plan, implement and review health improvement programmes in a range of settings.
Provide supervision, mentorship, and clinical support to students and junior staff in the team.
This is a full time post 37.5 hours per week. The post holder may be required to work flexibly to meet the needs of the service.

To provide leadership and work within the Integrated Community Rehabilitation Team, specifically, with adults who have been referred to the team, through provision of a high standard of physiotherapy specialist assessment, and core assessment details of other multidisciplinary skills.
Develop care and/or rehabilitation programme, to promote and sustain independence/well-being, for the individual or carer in the relevant setting, which may be a community, residential or health care environment.
To assess, plan, implement and evaluate treatment and care to people on an allocated caseload; promoting independence and autonomy; working within a multi-disciplinary team
Plan, implement and review health improvement programmes in a range of settings.
Recognise, assess and manage risk across the immediate and wider working environment ensuring statutory requirements are met.
To be responsible for patient safety through knowledge of systems, legal requirements and understanding of litigation.
Employment in this post requires an Enhanced/Standard Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.
