Job Summary
We are seeking two dedicated and compassionate Advanced Nurse Practitioner's to join our growing team. The successful candidate will provide high-quality patient care, support diagnosis and treatment plans, and work collaboratively with GP's and other healthcare professionals. The hours for the post are 30 hours per week.
Shift patterns are Wednesday , Thursday and Friday - 8:00am to 6:30pm.
Duties
- Conduct comprehensive patient assessments, including medical histories and physical examinations
- Diagnose illnesses and develop personalised treatment plans in collaboration with the healthcare team
- Provide health education to patients regarding their conditions and wellness strategies
- Ensure SNOMED codes are used effectively
- Request pathology services as necessary
- Process and interpret pathology and other test results as required
- Monitor patient progress and adjust treatments as necessary
- Maintain accurate and detailed medical records in compliance with legal and organisational standards
- Stay informed about the latest medical practices, guidelines, and innovations relevant to primary care
- Collaborate with multidisciplinary teams to ensure holistic patient care
Experience
- Proven experience working as an Advanced Nurse Practitioner
- Demonstrated competence in management of patient care plans
- Experience working with diverse patient populations is desirable but not essential
- Excellent communication skills, with the ability to build rapport with patients and colleagues alike
