Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.
We are looking for a compassionate and motivated Community Staff Nurse to join our Tunbridge Wells Community Nursing Team on a 28-hour per week basis. This rewarding role involves visiting housebound patients aged 18 and over in their own homes, delivering personalised nursing care to support those with complex and long-term conditions to remain safe, well and independent in their chosen environment. Working as part of a multidisciplinary team aligned to Primary Care Networks, you will undertake holistic assessments, plan and deliver care, provide health promotion and support self-management, and carry out a range of clinical interventions including wound care, medication administration, long-term condition management, and palliative and end-of-life care.

To support in the supervision and development of all junior staff including Health Care Support Workers and students.
Assessment, planning, implementation and evaluation of holistic care linked to packages of prescribed care.
Development of integrated care programmes in partnership with the patient
To Actively work as a member of the multi-professional team to provide high quality care to patients.
To undertake a range of competent clinical interventions to support the outcomes of both individual and group therapy including catheter care, administration of medication, wound care and end of life care
Manage a caseload of patients on a daily basis consisting of patients with complex needs associated within a specific speciality
