Full job description

The Role

You will be working alongside health care professionals supported by qualified Nurses and Physiotherapists working to their designed plans. You need to be technology literate or motivated to learn as you will be supporting your clients with the aid of TEC (Technology enabled care). We are looking for caring, compassionate people, who are organised and work on their own initiative. It is important that you have good communication skills in written and spoken English to join our team.

Responsibilities

Visiting the service users home
Working to care plans and Rehabilitation plans
Promoting and encouraging independence
Enabling Service Users to meet their outcomes and develop goals
Take part in MDT meetings
Personal Care
Companionship
