We are looking for an enthusiastic Social Worker to join our West Follow-On Team, based at Priory House in King’s Lynn. This is a varied and rewarding role supporting people following discharge from hospital, whether they are returning home or moving into care placements.
You will manage your own caseload, undertake Care Act assessments, complete Mental Capacity assessments and lead Section 42 safeguarding enquiries. The role requires confident decision-making, strong partnership working and a person-centred approach.
