Are you a registered Social Worker with experience of hospital discharge and integrated working?
Are you motivated by improving flow while also embracing new, strengths-based ways of working that support people to live well in their communities?
We are excited to offer a permanent Practice Consultant Social Worker role to work alongside our existing Practice Consultant and Team manager, providing professional leadership and oversight of social work practice within community hospital, health led recovery and reablement settings across West Norfolk and further afield.
This role sits at the heart of our developing approach, supporting safe and timely discharges, system flow, and the continued embedding of Community Led Support (CLS) across practice.
You will work closely with community hospitals, health partners and social care teams to ensure the right support is in place for people at the right time. Your role will include:
Providing day-to-day oversight of discharge activity and patient flow within community hospitals
Attending and contributing to daily operational and flow meetings, including multi-disciplinary forums
Supporting high-quality practice, including mental capacity considerations and safeguarding activity where required
Strengthening relationships across health, social care, voluntary and community partners
A key part of this role is supporting and championing our Community Led Support (CLS) approach. You will:
Promote strengths-based, person-centred practice, ensuring people’s networks, community assets and independence are considered from the point of admission and throughout discharge planning
Support practitioners and partners to think differently about risk, independence and proportionate interventions
Encourage a “home first” and community-focused mindset, aligning discharge planning with longer-term well-being
Contribute to service development and continuous improvement, helping embed CLS within integrated hospital pathways
