Team Leader

Location: Hunters Lodge Care Home, 39 Kiln Road, Fareham, Hampshire, PO16 7UQ
Salary: From £14.42 per hour
Hours: Full-time, 42 hours per week (8am-8pm)
Pattern: Two-week rolling rota, including alternate weekends
Contract Type: Permanent

The Role

We are looking for a compassionate, organised, and motivated Team Leader to join our dedicated care team. This is an exciting opportunity for an experienced care professional who is passionate about delivering outstanding person-centred care while leading and supporting others.

As a Team Leader, you will play a key role in ensuring the smooth day-to-day running of each shift, maintaining high standards of care, supporting staff development, and working closely with the management team to ensure compliance with regulatory requirements.

Key Responsibilities

Lead and coordinate the day-to-day running of the shift, ensuring safe, effective, and high-quality care is delivered at all times.
Administer and manage medication in line with company policies and current legislation.
Liaise professionally with GPs, district nurses, social workers, pharmacists, families, and other healthcare professionals to ensure residents receive the best possible care.
Provide guidance, support, and leadership to care staff throughout each shift.
Complete and update person-centred care plans, risk assessments, and daily documentation to ensure records remain accurate and compliant.
Support the management team with medication changeovers, stock checks, ordering, and medication audits.
Complete regular audits, identifying areas for improvement and implementing corrective actions where required.
Carry out staff supervisions, observations, and competency assessments to support learning, development, and compliance.
Assist with the induction and mentoring of new team members.
Promote a positive, inclusive, and supportive working environment.
Ensure safeguarding procedures, infection prevention, health and safety, and CQC standards are consistently upheld.
Respond appropriately to emergencies and ensure residents' wellbeing remains the highest priority.

About You

We are looking for someone who:

Has previous experience working within a residential or nursing care setting.
Has experience in a senior care or team leader role.
Holds a Level 3 Diploma in Health & Social Care.
Is confident administering medication and has a good understanding of medication management.
Has experience writing and reviewing care plans.
Demonstrates excellent leadership, communication, and organisational skills.
Is able to motivate and support colleagues to deliver exceptional care.
Has a good understanding of CQC regulations and best practice within adult social care.
Can work effectively under pressure while maintaining a calm and professional approach.
Is committed to providing compassionate, person-centred care.
